摘　　要：目的探讨尿动力学检测前列腺增生症(BPH)并发慢性尿潴留(CUA)患者膀胱功能的应用效果。方法回顾性分析150例BPH并发CUA患者术前尿动力学检查结果,150例患者依据术前检查结果和病情,给予相应治疗,如前列腺电切术及膀胱造瘘术等。结果 150例患者中低顺应性膀胱127例,高顺应性膀胱22例,顺应性正常的1例。膀胱逼尿肌收缩有力72例,均行前列腺电切术,术后皆获得满意治疗效果;膀胱逼尿肌活动低下49例,其中37例单纯行前列腺电切术,12例行前列腺电切术及耻骨上膀胱造瘘术;未见膀胱逼尿肌启动29例,其中22例行前列腺电切术及耻骨上膀胱造瘘术。13例患者为膀胱逼尿肌不稳定。结论术前尿动力学检查能评估BPH并发慢性尿潴留患者膀胱逼尿肌的顺应性及收缩功能,为手术方案的选择提供依据,合适的治疗方案可有效改善患者预后。Objective To investigate the effects of urodynamic test on the bladder function of patients with prostatic hyperplasia（ BPH） complicated with chronic urinary retention（ CUR）.Methods The preoperative urodynamic results of 150 patients with BPH complicated with CUA were retrospectively analyzed,and 150 patients received corresponding treatment based on preoperative examination results,such as prostatic electroectomy and bladder fistulectomy. Results There were 127 cases of low compliance bladder in 1 5 0 patients,2 2 cases of high compliance bladder and 1 case of normal compliance. And 72 patients with forceful bladder detrusor contraction were treated by prostatic electrocision,and the effects were better after the operation; there were 49 patients of detrusor bladder detrusor activity,of which 37 patients were treated by simple prostatic resection,and 12 patients were treated by prostatic electroectomy and suprapubic vesical fistulectomy; and there were 29 patients with no detrusor initiation of bladder,including 22 patients treated by prostatic electroectomy and suprapubic vesical fistulectomy,and 13 patients were detrusor instability of the bladder. Conclusions Preoperative urodynamic examination can evaluate the compliance and contractile function of the bladder detrusor in patients with BPH complicated with CUR,and it can provide a basis for selection of surgical procedures,effectively improve the prognosis of patients
