摘　　要：目的:临床药师参与合理用药管控的药学服务模式的建立。方法:医院建立合理用药考核及监管体制,制定《医师合理用药记分制管理考核标准》;通过信息化手段对重点监控药品的适应证进行管控,建立质控体系实时动态监测;临床药师针对不合理用药问题进行综合干预。结果:2017年上半年药占比、抗菌药物使用强度、重点监控药品费用、人均药费均呈下降趋势;各项指标均达标;且2017年上半年临床药师会诊数量明显增加。结论:临床药师在合理用药管控模式中发挥了重要作用,有助于合理用药各项指标的达标,促进临床合理用药。Objective： To establish a pharmaceutical service model for clinical pharmacists to participate in rational drug adminis- tration and control. Methods： A rational drug use assessment and supervision system was established, and the assessment criteria for the administration of rational drug use scoring system for physicians were developed in the hospital. Through the means of information technology, the indications of key monitoring drugs were controlled, and a real-time dynamic monitoring of quality control system was established. Finally, clinical pharmacists carried out comprehensive intervention with the problems in the irrational drug use. Results： Compared with that during the same period in 2016, the medicine proportion, antimicrobial use intensity, key monitoring drug costs and per capita drug costs showed a downward trend in the first half of 2017. All the indices were up to the standard. The number of clinical pharmacists participating in the clinical consultation increased significantly. Conclusion： Clinical pharmacists play an important role in the control mode for rational drug use, which is helpful to achieve the indicator standard of rational drug use, reduce drug abuse and promote clinical rational drug use.
