抗着丝点蛋白B抗体阳性的原发性干燥综合征的实验室特征分析

摘　　要：目的分析原发性干燥综合征(primary Sj9gren′s syndrome,pSS)抗着丝点蛋白B(CENP B),CENP B抗体阳性与抗CENP B抗体阴性的pSS病例实验室检查特点,为pSS的诊治提供更好的帮助。方法选取2013—2017年在北京大学口腔医院就诊的298例pSS患者的临床和实验室检查资料,分为抗CENP B抗体阳性组(87例)和抗CENP B抗体阴性组(211例)进行回顾性分析。结果抗CENP B抗体阳性的pSS患者发病年龄大于阴性组(64.20±10.67)岁vs(55.35±13.72)岁,差异有统计学意义(t=5.964,P<0.01)。血清ALP(95.32±56.78)U/L vs(74.16±22.78)U/L、GGT(74.05±157.33)U/L vs(27.43±40.98)U/L和IgM(2.04±1.85)g/L vs(1.50±1.17)g/L水平高于阴性组,AMAM2的检出率较高16.09%vs 8.06%,差异均有统计学意义(ALP:t=3.262,P<0.05;GGT:t=2.645,P<0.05;IgM:t=2.515,P<0.05;AMA-M2:χ2=4.27,P<0.05)。结论抗CENP B抗体阳性的pSS患者实验室检查特点与抗CENP B抗体阴性患者有差异,部分患者肝脏受累明显,可能是pSS中的一个类型。Objective To understand the laboratory features of primary Sjogren＇s syndrome（pSS）with positive,anti centromere protein B（CENP B）antibody by comparing to those with negative antibody in order to help for clinical diagnosis of pSS. Methods The clinical and laboratory data of 87 pSS with positive antiCENP B antibody and 211 pSS with negative anti-CENP B antibody were collected in School of stomatology,Peking university from 2013 to 2017 and a retrospective analysis was conducted. Results Patients with positive anti CENP B antibody were older than those with negative antibody（64.20±10.67 vs 55.35±13.72,t=5.964,P〈0.01）.Serum levels of ALP（95.32±56.78 vs 74.16±22.78）U/L,GGT（74.05±157.33 vs27.43±40.98）U/L and IgM（2.04±1.85 vs 1.50±1.17）g/L were all significantly higher among patients with positive antibody than those with negative one（P〈0.01）.Also,patents with positive antibody had higher positive rate of AMA-M2（16.09%vs 8.06%）,the difference was statistically significant（P〈0.05）.Conclusions The laboratory features of pSS patients with positive anti CENP B antibody differ from those with negative antibody,which might be associated with liver injures and could be one of the subtypes of pSS.
