120°儿童髋锁定加压钢板治疗DelbetⅣ型儿童股骨颈骨折

摘　　要：目的探讨120°儿童髋锁定加压钢板（120°LCP PHP）治疗DelbetⅣ型儿童股骨颈骨折的可行性和临床疗效。方法回顾性分析2009年6月至2014年10月收治的21例DelbetⅣ型股骨颈骨折患儿的临床资料。根据采用的内固定物不同将其分为PHP组及对照组。PHP组10例，采用120°LCP PHP固定；对照组11例，采用非LCP PHP钢板固定。比较两组手术时间、术中出血量、切口长度、切口愈合时间、骨折愈合时间、并发症、Haidukewych评分、Ratliff评分，并对结果进行分析。结果PHP组术后随访12～46个月，平均（23.2±2.1）个月；对照组术后随访13～42个月，平均（22.7±1.8）个月。两组在切口长度、切口愈合时间、Haidukewych评分、Ratliff评分方面差异无统计学意义（P〉0.05）。PHP组手术时间为（70.2±15.7）min，优于对照组（85.9±17.1）min；PHP组术中出血量为（62.2±12.7）ml，优于对照组（76.6±15.4）ml；PHP组骨折愈合时间为（13.2±2.0）周，优于对照组（16.0±1.6）周；PHP组术后并发症发生率为10%（1/10），优于对照组54.55%（6/11），组间比较，差异均有统计学意义（P〈0.05）。结论120°LCP PHP治疗DelbetⅣ型儿童股骨颈骨折，符合儿童股骨近端解剖形态及生物力学特点，具有手术时间及术中出血相对较少，固定牢固，骨折愈合快，并发症少及髋关节功能恢复良好等优点。ObjectiveTo explore the clinical efficacy and feasibility of 120° Locking compression pediatric hip plate （120°LCP PHP） for DelbetⅣ type femoral neck fractures in pediatric patients.MethodsA retrospective study was conducted for 21 children of DelbetⅣ type femoral neck fractures from October 2014 to June 2009. They were divided into PHP and control groups according to the type of internal fixtures. Ten cases in PHP group were fixed with 120 LCP PHP while 11 cases in control group received non LCP PHP plate. Two groups were compared with regards to operative duration, intraoperative blood loss, length of incision, wound healing time, fracture healing time, complications, Haidukewych score and Ratliff score.ResultsThe mean followed-up periods of PHP and control groups was （23.2±2.1） （12-46） and （22.7±1.8）（13-42） months respectively. Length of incision, wound healing time, Haidukewych score and Ratliff score of two groups had no significant difference. PHP group had shorter operative duration [（70.2±15.7）min vs. （85.9±17.1）min, P〈0.05], less intraoperative blood loss [（62.2±12.7）ml vs.（76.6±15.4）ml, P〈0.05], shorter fracture healing time [（13.2±2.0）weeks vs.（16.0±1.6）weeks, P〈0.05] and fewer complications [10%（1/10）vs. 54.5%（6/11）, P〈0.05]. And the differences were statistically significant.Conclusions120°Locking compression pediatric hip plate is efficacious for DelbetⅣ type femoral neck fractures in children.With proximal femoral anatomy and biomechanical characteristics of proximal femur in children, it has shorter operative duration, less intraoperative blood loss and shorter fracture healing time so as to reduce complications and effectively restore hip joint functions.
