摘 要:目的比较超微血流成像及超声造影两种方法在检测不同厚度的颈动脉斑块内新生血管的一致性,评估SMI在不同厚度颈动脉斑块新生血管中的应用价值。方法选择我院诊断的163例颈动脉粥样硬化患者,共计270个颈动脉斑块。所有患者行颈动脉常规超声检查,按斑块厚度分为三组,即≥3.5mm、2.5~3.5mm、1.6~2.5mm组。每组斑块均行SMI及超声造影检查,比较SMI和超声造影两种方法检测不同厚度斑块内微细血流的一致性。结果在≥3.5mm组及2.5~3.5mm组的斑块中两种检测方法一致性较好,但是在1.6~2.5mm组中两种检测方法具有一致性,但是一致性差。结论 SMI是检测颈动脉斑块内新生血管安全有效的检查方法,对评估颈动脉斑块稳定性具有重要价值,在一定程度上可以替代超声造影检查。Objective Comparison of diagnostic values of the superb micro-vascular imaging and contrast-en hanced ultrasound in carotid plaques for neovascularization with different thicknesses, to evaluate the application of SMI in carotid artery plaques. Methods One hundred and sixty-three patients and two hundred and seventy carotid ar- tery plaques were studied from. All the patients were examined by routine carotid artery ultrasonography. All carotid artery plaques were divided into three groups according to the thickness of the plaques, namely,≥3.5 mm, 2.5--3.5 mm, 1.6--2.5 mm. Each group was examined by SMI and CEUS, to compare the detection sensitivity of the micro blood flow in the plaques with different thicknesses. Results The two methods have good consistency in the plaques with the thicknesses of 93.5 mm and 2.5--3.5 mm. But in the 1.6--2.5 mm group, the two methods are consistent, but the consistency is not ideal. Conclusions SMI can be safely and effectively detected neovascularization in carotid artery plaque. SMI is important to evaluate the stability of carotid artery plaques, and to some extent, it can be re- placed the CEUS.
