温养督脉法治疗腰椎间盘突出症伴发心理障碍患者疗效及对HAMD-24量表评分的影响研究

摘　　要：腰椎间盘突出症该病的临床表现除腰痛及下肢放射痛外,具有的慢性发病、病程较长的特点,严重影响患者的劳动能力和生活质量,从而产生焦虑、抑郁等心理障碍,严重影响生活质量。对接受住院治疗的64例腰椎间盘突出症患者,使用补肾壮督方治疗抑郁和焦虑为特点的心理障碍的临床研究,治疗组口服补肾壮督方;对照组口服氟西汀胶囊。两组均配合常规治疗。两组患者治疗前后HAMD-24量表评分比较,中药组和对照组在治疗前不存在统计学的差异（P〉0.05）,在治疗2周后也不存在统计学的差异（P〉0.05）,治疗前后两组患者Oswestry功能障碍指数问卷调查表（ODI）得分比较,治疗前两组患者ODI评分比较差异无统计学意义（P〉0.05）。治疗1周后两组患者ODI得分比较（P0.05）无统计学意义,1周后中药组优于对照组。2周后未见明显差异。总体来讲中药治疗组较对照组分析改善腰腿痛症状,治疗短时间内要好于西药组。但中药不良反应相对要少,患者依从性好。The clinical manifestation of lumbar disc herniation is not only low back pain and lower limb radiation pain,but also the characteristics of chronic disease and long course,seriously affecting the labor ability and the quality of life of the patients.Sixty-four cases of lumbar disc herniation patients in our hospital who recieved Bushen Zhuangdu Formula in the treatment of mental disorders characterised with depression and anxiety were studied. The treatment group were treated with Bushen Zhuangdu Formula and patients in control group were treated with fluoxetine hydrochloride capsule. The two groups were treated with conventional therapy. Two groups＇ HAMD-24 score comparison before and after treatment showed the two groups had no difference before treatment（P〉 0. 05） and there was no statistical differences between two groups after 2 weeks treatment（P〉 0. 05）. The two groups＇ Oswestry Disability Index questionnaire（ODI） scores before and after treatment had no significant difference before treatment（P〉 0. 05）. After 1 week treatment,the ODI score in the treatment groupwas statistically significant（P〉 0. 05）. After 2 weeks treatment,the scores of the ODI in the two groups had no statistical significance（P〉 0. 05）. In general,Chinese medicine treatment group is superior to the control group in improving the symptoms of low back pain,for a short period of time is better than western medicine group. However,the side effects of traditional Chinese medicine are less,and the patients＇ compliance is good.
