“多元性辅助用药预警监控体系”在药品管理中的实践

摘　　要：目的:加强辅助用药的管控,促进医院合理用药。方法:建立多元性辅助用药预警监控体系,评价管理前后辅助用药使用的变化。结果:管理后某院辅助用药(注射剂型)品种数及品规数均明显减少,辅助用药销售金额较之前下降了16.16%;医院药占比由管理前的40.75%下降至35.98%;辅助用药销售占比由35.46%下降至31.32%。但全院销售前20位药品中辅助用药仅由12种减少为8种,辅助用药所占比重仍然较高;人均总药费由管理前的5 000元左右下降到3 700元以下,人均辅助用药费用也平均下降了约600元。结论:加强辅助用药的管控,能降低药品费用,减轻患者经济负担,促进合理用药;但该项管理工作仍存在难点,应进一步加强对临床科室全覆盖管控及对临床医师和患者的宣教。OBJECTIVE To strengthen the control over adjuvant drugs and promote the rational use of drugs.METHODSA diverse adjuvant drug early warning and monitoring system was constructed to evaluate changes in the use of adjuvant drugs before and after management.RESULTS After management,the varieties and specifications of adjuvant drugs（injections）in our hospital decreased significantly.The sales volumes of adjuvant drugs decreased by 16.16%.The proportion of hospital drugs fell from 40.75%to 35.98%.The proportion of adjuvant drugs fell from 35.46%to 31.32%.But among Top 20 drugs sold in the hospital,the varieties of adjuvant drugs only dropped from 12 to 8.The proportion of adjuvant drugs was still high.The per capita drug cost dropped from about 5 000 yuan to less than 3 700 yuan.The per capita adjuvant drug cost also dropped by about 600 yuan on average.CONCLUSION Strengthening the management of adjuvant drugs is of great significance for promoting rational clinical drug use,increasing medication safety,controlling drug costs and reducing financial burdens on patients.But there are still difficulties in this job,and measures are needed to strengthen full coverage and control over clinical departments,as well as publicity and education among clinicians and patients.
