中药联合电针治疗脊柱骨折合并脊髓损伤的指标分析

摘　　要：目的观察中药与督脉电针对脊柱骨折合并脊髓损伤患者的Frankel评分、运动功能评分(MS评分)的影响。方法选取我院收治的脊柱骨折合并脊髓损伤患者83例,根据治疗方法不同分为对照组和观察组。两组患者均进行手术减压、骨折复位内固定治疗,术后均采用我院常规护理,对照组在此基础上联合中药治疗,观察组在对照组治疗基础上联合督脉电针治疗。比较两组手术前后因子水平变化情况,治疗后患者脊髓损伤恢复情况、运动功能恢复情况及括约肌功能变化情况。结果两组手术7d后C反应蛋白(CRP)和降钙素原(PCT)水平均较治疗前上升,观察组手术7d后CRP和PCT水平低于对照组(P<0.05);两组手术7d后D二聚体(D-D)、血小板计数(PLT)及纤维蛋白原(FIB)水平均较治疗前上升,观察组手术7d后D-D、PLT及FIB水平低于对照组(P<0.05);两组治疗半年后疼痛评分和括约肌功能评分较治疗前下降,运动功能评分均较治疗前上升(P<0.05);观察组治疗半年后疼痛评分、括约肌功能及运动功能评分优于对照组(P<0.05),两组治疗半年后神经功能FrankeL分级分布情况均较治疗前改善,观察组治疗半年后神经功能Franke分级分布情况优于对照组(P<0.05)。结论中药联合督脉电针治疗脊柱骨折合并脊髓损伤患者疗效确切,可有效控制患者手术初期炎症因子和凝血指标水平,改善患者运动功能和括约肌功能。Objective To observe the effects of traditional Chinese medicine （TCM） combined with electro-acupuncture （EA） on MS score, Frankel score of spinal fracture combined with spinal cord injury. Methods 83 cases of patients with spine fracture combined with were divided into observation group and control group due to the different treatment methods. Both groups were treated with surgical decompression, reduction and internal fixation of fracture and routine nursing, control group plus TCM treatment, observation group received the combination of Goremor vessel EA treatment on the basis of control group. The level changes of the two groups before and after the operation were observed and compared. The recovery of spinal cord injury, the recovery of motor function, the changes of sphincter function after treatment in were compared. Results CRP and PCT levels of both groups after 7d of operation were higher than those before treatment, the level of treatment group was lower than that of control group （P〈0.05）, D-D, PLT and FIB levels of both groups were higher than before treatment, observation group were lower than those of control group （P〈0.05）. After 6 months of treatment, the pain score and sphincter function score decreased in both groups, while motor function score significantly increased （P〈0.05）, which of observation group were better than that of control group （P〈0.05）.The Frankel scores of distributed hierarchical neural function 6 month after treatment were improved compared with before treatment in both groups, which of observation group was better than that of control group （P〈0.05）.Conclusion The combination of TCM and EA is effective in treatment of spinal fracture, which can effectively control the early inflammatory factors and coagulation index level, thus to improve the motor function and sphincter function.
