排石冲剂对体外冲击波碎石术后尿路结石的疗效观察

摘　　要：目的观察排石冲剂治疗经体外冲击波碎石(ESWL)术后尿路结石的疗效。方法将符合纳入标准的64例尿路结石患者随机分为对照组和治疗组,每组32例。两组行ESWL术后,对照组给予解痉、止痛等治疗,治疗组在对照组治疗的基础上配合排石冲剂治疗。观察28天后两组患者临床症状、体征,判断疗效及结石复发率;检测治疗前后两组患者尿NAG、尿β2-微球蛋白、尿钙及血肌酐、血胱抑素C的水平。结果治疗组总有效率明显优于对照组(P<0.05),治疗组结石复发率明显低于对照组(P<0.05);治疗组治疗后尿NAG、尿β2-微球蛋白、尿钙、血清胱抑素C水平较对照组均有显著下降(P<0.05),但血肌酐较对照组无显著性差异(P>0.05)。结论尿路结石ESWL术后患者口服排石冲剂,能明显改善疗效,促进结石的排出,减少结石的复发,减轻急性肾损伤,其机理可能与排石冲剂能减少尿钙的排泄,减轻ESWL对肾小管的损伤有关。Objective To observe the effects of Paishi granule on urinary calculi after extracorporeal shock-wave lithotripsy （ ESWL ）. Methods 64 cases of patients with urinary calculi were divided into control group and treatment group, 32 cases in each group. After ESWL, control group was given conventional treatment, treatment group was given Paishi granule on the basis of control group, 28d was a course of treatment. The clinical symptoms and urinary n-acetyl-β-d-glucosidase （NAG）, urine beta 2-microglobulin （β2-MG） and urinary calcium and serum creatinine, serum cystatin C levels were observed. Results The total effective rate of treatment group was significantly higher than that of control group （P〈0.05）, and the recurrence rate of stones in treatment group was significantly lower than that in control group （P〈0.05）. After treatment, urine NAG enzyme β2 - microglobulin, urinary calcium, urinary inhibition of serum C level of treatment group were significantly decreased （P〈0.05）, but there was no significant difference in serum creatinine compared with control group （P〉0.05）. Conclusion After taking ESWL for urinary calculi, oral Paishi granule can significantly improve the curative effect, promote the excretion of calculus, reduce the recurrence of stones, and reduce the acute kidney injury. The mechanism may be related to the excretion of urinary calcium and the reduction of ESWL on renal tubular injury.
