氯沙坦联合骨化三醇治疗糖尿病肾病对24 h尿微量白蛋白、肌酐、尿酸的影响

摘　　要：目的探究在糖尿病肾病患者中应用氯沙坦联合骨化三醇治疗对24 h尿微量白蛋白、肌酐、尿酸产生的影响。方法对2016年1月—2017年1月该院收治的72例糖尿病肾病患者随机平均分成两组进行研究,对照组行以氯沙坦治疗,研究组行以氯沙坦联合骨化三醇治疗,对两组治疗前后24 h尿微量白蛋白、肌酐以及尿酸水平进行分析和比较。结果研究组患者的空腹血糖值、糖化血红蛋白明显比对照组低(P<0.05),而两组患者的尿素氮水平、胆固醇水平、甘油三酯水平、谷丙转氨酶水平、谷草转氨酶水平比较差异无统计学意义(P>0.05)。治疗前,两组患者的24 h尿微量白蛋白、肌酐以及尿酸水平均无显著差异,治疗后两组患者这3项指标水平均明显降低,组间差异有统计学意义(P<0.05);治疗前两组患者血压水平均无显著差异,治疗后两组患者的血压水平均明显下降,组间差异无统计学意义(P>0.05)。结论在糖尿病肾病患者中应用氯沙坦联合骨化三醇治疗可以有效控制24 h尿微量白蛋白、肌酐以及尿酸水平,降低血压,疗效显著。Objective To study the effect of losartan combined with calcitriol in treatment of diabetic nephropathy on the24 h urine microalbumin, creatinine and uric acid. Methods 72 cases of patients with diabetic nephropathies admitted and treated in our hospital from January 2016 to January 2017 were selected and randomly divided into two groups, the control group were treated with losartan, while the research group were treated with losartan combined with calcitriol, and the 24 h urine microalbumin, creatinine and uric acid of the two groups before and after treatment were analyzed and compared. Results The fasting blood glucose value and glycosylated hemoglobin levels in the research group were obviously lower than those in the control group（P 0.05）, and the differences in the urea nitrogen, cholesterol, triglyceride, glutamic-pyruvic transaminase, and glutamic-oxalacetic transaminase levels between the two groups were not obvious（P0.05）, before treatment, there were no obvious differences in the 24 h urine microalbumin, creatinine and uric acid between the two groups,after treatment, the levels of three indicators obviously decreased, and the differences were obvious（P0.05）, before treatment, there was no obvious difference in the blood pressure level between the two groups before treatment, and the blood pressure level of the two groups after treatment obviously decreased, and difference was not obvious（P0.05）. Conclusion The losartan combined with calcitriol in treatment of diabetic nephropathy can effectively control the 24 h urine microalbumin, creatinine and uric acid, and reduce the blood pressure, and the curative effect is obvious.
