摘 要:目的探讨双源CT头颈部血管成像对急性脑梗死患者颈动脉狭窄的评估作用。方法选取我院2016年11月~2017年11月收治的70例急性脑梗死患者作为研究对象,在征得患者家属知情同意下实施双源CT头颈部血管成像检查并以数字减影血管造影为参照标准,探讨双源CT头颈部血管成像对急性脑梗死患者颈动脉狭窄的评估作用。结果数字减影血管造影结果证实,70例急性脑梗死患者中无狭窄5例、轻度狭窄8例、中度狭窄35例、重度狭窄14例、完全闭塞8例,双源CT头颈部血管成像诊断符合率分别为100.00%、100.00%、94.74%、92.86%、100.00%,差异无统计学意义(P>0.05);双源CT头颈部血管成像评估急性脑梗死患者颈动脉狭窄的敏感度95.00%、特异度100.00%、准确率95.71%,与数字减影血管造影相比较,差异无统计学意义(P>0.05)。结论在急性脑梗死患者颈动脉狭窄评估工作中双源CT头颈部血管成像可收获理想的评估结果,值得于今后临床工作中推广使用。Objective To evaluate the assessment role of head and neck dual-source CT angiography in carotid stenosis in patients with acute cerebral infarction. Methods Seventy patients with acute cerebral infarction admitted from November 2016 to November 2017 in our hospital were enrolled in this study. Head and neck dual-source CT angiography was performed under the informed consent of their relatives. The assessment of head and neck dual-source CT angiography in carotid artery stenosis in patients with acute cerebral infarction was explored, with digital subtraction angiography as a reference standard. Results The results of digital subtraction angiography confirmed that there were 5 cases without stenosis, 8 cases of mild stenosis, 35 cases of moderate stenosis, 14 cases of severe stenosis and 8 cases of totally occlusion in 70 cases of acute cerebral infarction. The coincidence rate of head and neck dual-source CT angiography was 100.00%, 100.00%, 94.74%, 92.86% and 100.00% respectively, with no significant difference(P〉0.05).The sensitivity, specificity and accuracy was 95.00%, 100.00% and 95.71% of head and neck dual-source CT angiography in evaluating carotid stenosis in patients with acute cerebral infarction, and the difference was not statistically significant compared with digital subtraction angiography(P〉0.05). Conclusion Head and neck dual-source CT angiography can gain ideal assessment results in the evaluation of carotid stenosis in patients with acute cerebral infarction,which is worth to be popularized in future clinical work.
