摘 要:目的探讨前列地尔脂微球载体制剂(Lipo PGE1)对慢性阻塞性肺疾病(Chronic obstructive pulmonary disease,COPD)患者肺功能及呼出冷凝液(Exhaled breath condensate,EBC)中内皮素-1(Endothelin-1,ET-1)、白介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)的影响。方法随机选取2016年1-12月我院收治的120例明确诊断为COPD急性加重期的患者,采用抛硬币法平均分为对照组及观察组,每组60例,对照组采取平喘、化痰、解痉、吸氧、抗炎等常规治疗,观察组在常规治疗基础上加以Lipo PGE1治疗,比较治疗前后两组患者的血气指标、肺功能指标及EBC中ET-1、IL-6、TNF-α含量的变化情况。结果治疗前,两组PaCO_2、PaO_2、FEV1/预计值、FEV1/FVC、ET-1、IL-6、TNF-α比较,差异无统计学意义(P>0.05)。治疗后,两组患者PaCO_2、ET-1、IL-6、TNF-α均显著低于治疗前,且观察组显著低于对照组,差异有统计学意义(P<0.05);两组患者PaO_2、FEV1/预计值、FEV1/FVC均显著高于治疗前,且观察组显著高于对照组,差异有统计学意义(P<0.05)。结论前列地尔脂微球载体制剂可有效改善COPD患者的肺功能,并能有效降低EBC中ET-1、IL-6、TNF-α水平。EBC中ET-1、IL-6、TNF-α水平可作为COPD疗效的判断指标,具有重要的临床意义。Objective To investigate the effect of Lipo PGEI on pulmonary function and ET-I, IL-6 and TNF- α in the exhaled breath condensate (EBC) in patients with chronic obstructive pulmonary disease (COPD). Methods Totally 120 patients with COPD were enrolled in this study, who were treated in our hospital from January to Decem- ber 2016. The patients were randomly divided into control group (n -60) and observation group (n -60) according to the random principle. Control group was treated with antiasthmatic, phlegm reducing, anti-spasmodic, oxygen inhala- tion, anti-inflammatory and other routine treatment. Observation group was given Lipo PGEI on the basis of convention- al therapy. The changes of blood gas index,pulmonary function index and the contents of ET-I, IL-6 and TNF-α in EBC in the two groups were compared before and after treatment. Results There was no significant difference in PaCO2 , PaO2 , FEVl/prediction values, FEVI/FVC,ET-I, IL45 or TNF-α between the two groups ( P 〉 0. 05 ) before treatment. After treatment, the PaCO2 ,ET-I, IL-6 and TNF-α in the two groups were decreased significantly (P 〈 0. 05) , while PaO2 ,FEVl/predicted values and FEVI/FVC were increased significantly, there was significant differ- ence between the two groups ( P 〈 0. 05 ). Conclusion Prostaglandin microsphere carrier preparation can effectively improve the lung function of patients with COPD,and can effectively decrease the levels of ET-1, IL45 and TNF-α in EBC. The levels of ET-I ,IL-6, and TNF-α in EBC can be used as indicators for the curative effect of COPD, and it has important clinical significance.
