摘 要:目的观察右美托咪定预给药对切口痛模型大鼠脊髓胶质细胞的影响。方法成年雄性SD大鼠60只,随机分为4组(n=15):S组为假手术组,O组为手术对照组,D1、D2组分别为低剂量及高剂量右美托咪定组。O组、D1组和D2组大鼠行左后足切开术制备切口痛模型,D1、D2组于术前10 min分别腹腔注射右美托咪定30μg/kg及50μg/kg。S组和O组腹腔注射等量生理盐水。于建立切口痛模型前(T0)及术后2、6、12、24 h(T1-4)观察机械缩足反射阈值(MWT),于行为学测试结束后每组随机取6只大鼠,取L4-6脊髓进行Western blot方法测定小胶质细胞标记物(OX42)和星形胶质细胞标记物(GFAP)的蛋白表达水平及测定IL-1β、IL-6和TNF-α含量。结果与S组比较,O组大鼠T1-3时MWT值下降,脊髓OX42表达上调,脊髓IL-1β、IL-6和TNF-α含量升高(P<0.05);与O组比较,D1组及D2组大鼠T1-3时MWT值升高,脊髓OX42表达下调,脊髓IL-1β、IL-6和TNF-α含量降低(P<0.05)。结论在切口痛模型中,右美托咪定主要抑制大鼠脊髓小胶质细胞OX42的表达及炎性因子IL-1β、IL-6和TNF-α释放。Objective To observe the effcet of dexmedetomidine on spinal glial cells in rat model of incisional pain. Methods Sixty male SD rates were randomly divided into four groups (n = 15) : group S (sham operation group), group O (operation group), group D1 (dexmedetomidine low-dose group) and group D2 (dex- medetomidine higher dose group ). Group O, group D1 and group D2 were used to establish the model of incisional pain. The rats in group D1 were given intraperitoneal injection of dexmedetomidine (30 ug/kg), group D2 were giv- en intraperitoneal injection of dexmedetomidine (50 g/kg) , group S and group O were given with equal volume of saline. Mechanical withdrawal threshold ( MWT ) were measured before operation ( To ) and 2 h, 6 h, 12 h, 24 h ( T1-4 ) after operation. L4-6 spinal cord was obtained immediately after the behavioral tests from 6 rats in each group, which were randomly assigned. The expression of OX42 in spinal mieroglial cell and GFAP in spinal astrocyte were detect- ed by immunohistochemistry. IL-1 β, IL-6 and TNF-α expression were detected by ELISA. Results Compared with group S, MWT decreased in group O at T1-3, the expression of OX42, IL-1β, IL-6 and TNF-α, increased in spinal cord (P 〈 0.05 ) ; compared with group O, MWT increased in group D1 and group D2 at T1-3, the expression of OX42, IL-1β, IL-6 and TNF-α, decreased in spinal cord (P 〈 0.05 ). Conclusion In rat model of ineisional pain, dexmedetomidine could suppress the expression of OX42 in spinal microglial cell and the release of IL- 1 β, IL-6 and TNF-α.
