摘 要:目的研究连续5天反复体位改变对人体头低位耐力的影响及持续时间。方法 17名健康男性进行连续5天的HDT/HUT模式反复体位改变训练,训练前1天,训练后第1,7,14,28天分别进行头低位耐力检查,监测受试者主观感觉与体征、血压、心功能、外周循环阻力(total peripheral resistance,TPR)和唾液皮质醇变化情况。结果与实验前1天相比,实验后第1,7,14,28天头低位耐力检查头低位主观感觉与体征评分均有提高,但无显著性。历次检查平卧位收缩压(systolic blood pressure,SBP)、舒张压(diastolic blood pressure,DBP)、平均压(mean arterial pressure,MBP)和TPR均无显著变化。实验后第1天头低位SBP、DBP和MBP在各时段均显著降低(P<0.05或P<0.01),第7天SBP在第1个10 s,DBP和MBP在第1,2,3个10 s仍有下降趋势,但无显著性。实验后第1、7和28天平卧位SV显著降低(P<0.05),其他心功能各指标变化不显著。实验后第1天头低位各时段TPR均显著降低(P<0.05);第7天头低位TPR仅在第2个10 s时低于实验前(P<0.05),第14天TPR较实验前已无显著性改变,第28天头低位TPR在第2个10 s显著升高(P<0.05)。各次皮质醇均无显著差别。结论连续5天HDT/HUT反复体位改变训练可以改善受试者头低位主观感受和体征,能通过降低血压改善机体头低位耐力水平;可能是通过降低外周循环阻力来降低头低位耐力时血压,效果可维持1周左右。Objective To verify the influence of a 5-day repeated body position change training on human headdown tilt( HDT) tolerance,and how long it will maintain. Methods Seventeen young healthy male subjects were trained with repeated position change once a day for 5 days. The HDT tolerance tests were carried out on the day immediately before the first training,the 1 st day,the 7 th day,the 14 th day and the 28 th day after the last training respectively,during which the subjective feelings and the symptoms,blood pressure,some cardiac parameters,the total peripheral resistance( TPR) and the salivary cortisol were monitored. Results Compared with the day before the first training,scores of subjects' sensation and symptom during the HDT tests after training increased but with no significance. The systolic blood pressure( SBP),diastolic blood pressure( DBP) and mean arterial pressure( MBP) during HDT test on the first day after training decreased significantly( P 〈 0. 05,P 〈 0. 01). The first 10 s of the SBP during HDT test,the three 10 s of the DBP and MBP during HDT test on the 7 th day after training decreased but without significance. The supine SV on the 1 st,7 th,and 28 th day after training decreased significantly( P 〈 0. 05),while changes of other cardiac parameters were not significant. TPR during HDT test on the 1 st day after training decreased significantly( P 〈 0. 05). TPR during HDT on the 7 th day after training decreased,but only of significance in the second 10 s( P 〈 0. 05).TPR during HDT test on the 14 th day changed insignificantly. The second 10 s of TPR during HDT test on the28 th day after training increased significantly. All the salivary cortisol changed insignificantly. Conclusion Five days repeated body position change training could improve the subjective sensations and the symptoms during HDT test.
