瑞巴派特治疗幽门螺杆菌阳性胃溃疡90例的疗效及细胞因子变化

摘　　要：目的:研究瑞巴派特治疗幽门螺杆菌(Helicobacter pylori,Hp)阳性胃溃疡患者的细胞因子变化。方法:经胃镜及病理检查和快速尿素酶试验确诊为Hp感染的胃溃疡患者共90例,随机分为三联组、铋剂四联组和瑞巴派特四联组。三联组给予泮托拉唑、阿莫西林和克拉霉素治疗;铋剂四联组在上述三联药物的基础上加用铋剂;瑞巴派特四联组在三联药物的基础上加用瑞巴派特。治疗2周停用抗生素,所有患者疗程为4周,治疗前后均完善胃镜及细胞因子检查。结果:瑞巴派特四联组比三联组4周愈合更好,结果具有显著性差异。瑞巴派特四联组IL-1,IL-6和MDA和其他两组比较均明显降低,而EGF,PGE_2和NO,VEGF均明显升高,差异具有统计学意义(P<0.05)。结论:瑞巴派特可以抑制Hp产生的侵袭因子,并增加保护因子的合成而起到保护胃黏膜的作用。Objective： To analyze the change of cytokines under rebamipide in the patients with Helicobacter pylori（ Hp） positive gastric ulcer. Methods： 90 patients with Hp positive gastric ulcer who were diagnosed by the endoscopy,pathology test and rapid urease test,were divided into three groups randomly which are triple group,bismuth quadruple group and rebamipide quadruple group. The triple group took pantoprazole,amoxicillin and clarithromycin; while besides the three medicine,the bismuth quadruple group added bismuth and the rebamipide quadruple group added rebamipide. After two weeks＇ treatment,antibiotics were discontinued. All patients were treated for 4 weeks,and they all received endoscopy and cytokines test before and after the treatment. Results：Compared with the triple group, the rebamipide quadruple group get a better treatment effect significantly.Compared with the other two groups,the concentration of IL-1,IL-6 and MDA of the rebamipide quadruple group were significantly lower and the concentration of the EGF,PGE2,NO and VEGF were significantly higher（ P〈0. 05）. Conclusion： The anti-ulcer and mucosa protection effects of rebamipide are contribute to the stimulation of defensive factors and inhibition of attack factors induced by Hp.
