静注伊布利特转复房颤射频消融中的安全性及有效性

摘　　要：目的探讨静注伊布利特转复心房颤动(房颤)射频消融术中的有效性及安全性。方法 200例行房颤射频消融患者,射频消融术后仍为房颤者,给予静注伊布利特,转为窦律为成功;转复失败者中仍为房颤者给予电除颤,转为房扑、房速者给予相应消融。观察开始给药30 min中内伊布利特转复率及4 h不良反应。结果 (1)伊布利特转复房颤消融术后仍为房颤者的转复率为81%。(2)伊布利特注射后30min的QTc间期(0.48±0.07)s明显长于用药前的QTc间期(0.41±0.18)s,P<0.01;术前EF(55.20±3.88)%,术后测EF(54.17±4.91)%,差异无统计学意义P>0.05。无恶性心血管事件发生。结论静脉注射伊布利特针对房颤射频消融术后仍为房颤者转复是安全有效的。Objective To expore the efficacy of ibutilide for cardioversion of atrial fibrillation during radiofrequency abla- tion. Methods 200 cases with atrial fibrillation underwent the radiofrequency ablation. The cases who were still atrial fibralltion of the operation accepted intravenous injection of ibutilide. The failure who were still atrial fibralltion after the drug were given electric defibrillation. The rest who turned into atrial flutter or atrial tachycardia underwent correspond- ing radiofrequency ablation. Cardioversion rate of ibutilide 30 min after treatment and severe adverse reactions within 4h were analyzed. Results Cardioversion rate of ibutilide is 81% for ihe cases who were still atrial firallation after the ra- diofrequency ablation. The QTc interval l0. 48 ~ 0.07} s at 30min after using ibutilide was significantly longer than before injection { O. 41 ~ 0. 18 ） s. The left ventrieular function had no significant differences between before and after operation. There was no malignant cardiovascular events whthin 4h after intravenous injection of ibutilide. Conclusion Intrave- nous injection of ibutilide was effective and safe to atrial firallation after the radiofrequency ablation.
