中药内服外用治疗膝关节滑膜炎(湿热阻络证)的临床研究

摘　　要：目的观察加味二妙散口服联合金黄散外敷治疗膝关节滑膜炎(湿热阻络证)的临床疗效并探讨其作用机制。方法将70例患者随机分为对照组和治疗组各35例。对照组予以塞来昔布胶囊口服联合功能锻炼治疗,治疗组在对照组基础上予以加味二妙散口服、金黄散外敷,疗程均为4周。比较两组治疗前后膝关节VAS评分、肿胀度评分、WOMAC评分、临床疗效及不良反应。结果治疗后治疗组膝关节VAS评分、肿胀度评分及WOMAC评分,与本组治疗前及对照组同期比较均降低(P<0.05);治疗组治疗有效率为94.29%,明显高于对照组74.29%(P<0.05)。对照组有2例患者出现恶心、干呕等不良反应,其余患者均未出现不良反应。结论加味二妙散口服联合金黄散外敷可有效促进膝关节滑膜炎患者膝关节肿胀、疼痛等症状缓解,有助于患膝功能恢复,且不良反应少。Objective： To observe the clinical curative effect of oral and external application of traditional Chinese medicine on gonarthromeningitis with damp-heat blocking collaterals. Methods： 70 patients were randomly divided into the control group（35 cases） and the treatment group（35 cases）. The control group was treated with celecoxib capsules and functional exercise. The treatment group was treated with Modified Ermiao Decoction and Jinhuang Decoction. The treatment lasted for 4 weeks. VAS score and swelling degree score of the knee joint,WOMAC score,clinical efficacy and adverse reactions were compared between the two groups before and after treatment. Results： After treatment,the VAS score,swelling score and WOMAC score of the knee joint in the treatment group were significantly lower than those in the control group（P 〈 0.05）. The effective rate was 94.29%（33/35） in the treatment group,higher than that in the control group 74.29%（26/35）;the difference was statistically significant（P 〈 0.05）. There were 2 patients with nausea,retching and other adverse reactions in the control group,and the rest had no adverse reactions. Conclusion： Modified Ermiao Decoction combined with Jinhuang Decoction can effectively relieve swelling and pain of the knee joint in gonarthromeningitis,helpful to restore knee function,with less adverse reaction.
