自拟解毒止痛汤联合贴棉灸治疗带状疱疹(急性期)的疗效及对后遗神经痛发生率的影响

摘　　要：目的观察采用自拟解毒止痛汤联合贴棉灸治疗带状疱疹(急性期)的临床疗效及对后遗神经痛发生率的影响并探讨其机制。方法 128例患者按就诊顺序随机分入联合组与基础组各64例。基础组给予相应的抗病毒、营养神经、调节免疫力等治疗,联合组在除基础组治疗外,另予自拟解毒止痛汤并贴棉灸疗法;两组治疗均10d为1个疗程。结果联合组止疱与结痂时间均短于基础组(均P<0.05)。治疗后两组视觉模拟量表(VAS)评分与治疗前比较均改善(均P<0.01),且联合组评分相较于基础组降低(P<0.05)。治疗后两组血清中白细胞介素-6(IL-6)和白细胞介素-10(IL-10)与治疗前比较,均有改善(P<0.05或P<0.01);且联合组改善程度均优于基础组(均P<0.05)。联合组总有效率为100.00%,高于基础组的70.31%(P<0.05)。联合组后遗神经痛发生率为9.38%,低于基础组的45.31%(P<0.05)。结论采用自拟解毒止痛汤联合贴棉灸治疗带状疱疹(急性期)的临床应用疗效显著,且能够有效降低后遗神经痛的发生率。Objective： To investigate the clinical efficacy of self-made Jiedu Zhitong Decoction combined with cotton moxibustion on herpes zoster（acute stage） and its influence on the incidence of postherpetic neuralgia.Methods： 128 patients were randomly divided into the combined group and the basic group according to the order of treatment,64 cases in each. The patients in the basic group were given corresponding antiviral,nutritional nerve and immune regulation treatment. The combined group was added to the self-prescribed Jiedu Zhitong Decoction combined with cotton moxibustion therapy in addition to the basic group treatment. The treatment course lasted for 1 course 10days. Results： The time of blisters and scab in the combined group was lower than that of the basic group（all P 〈 0.05）. Before treatment,there was little difference in VAS score between the two groups（P 0.05）. After treatment,the VAS scores of the two groups were improved compared with those before treatment（P 0.01）,and the score of the combined group was lower than that in the basic group（P 〈 0.05）. Before treatment,the levels of serum IL-6 and IL-10 in the two groups were not very different（P 〉 0.05）. After treatment,the serum levels of IL-6 and IL-10 in the two groups were improved（P 〈 0.05 or P 〈 0.01）,and the degree of improvement in the combined group was better than that in the basic group（P 〈 0.05）. The total effective rate of the combined group was 100%,higher than that of the basic group（P 〈 0.05）. The incidence of neuralgia in the combined group was 9.38%,lower than that of the basic group（45.31%）（P 〈 0.05）. Conclusion： The clinical application of selfmade Jiedu Zhitong Decoction combined with cotton moxibustion on herpes zoster（acute stage） has a significant curative effect,and can effectively reduce the incidence of sequela of neuralgia.
