壮医热敏探穴针刺疗法结合壮药龙钻通痹方治疗类风湿关节炎的临床观察

摘　　要：目的：通过科学、规范化的临床研究,客观评价壮医热敏探穴针刺疗法结合壮药龙钻通痹方治疗类风湿关节炎的临床疗效及安全性。方法：遵循简单随机、平行对照、非盲的方法。首先按照性别对纳入对象进行分层,再应用统计软件SPSS 22.0进行随机数字分组法,将符合纳入标准的64例受试患者,按照1：1的比例,随机分为两组。对照组32例给予西药常规治疗,试验组32例在对照组的治疗基础上加用壮医热敏探穴针刺疗法结合壮药龙钻通痹方治疗,经过相同时间的治疗后,综合分析两组治疗RA的临床疗效。结果：经4周治疗后,最终60例患者完成临床试验。试验组达到2011年缓解标准的26例,未达到缓解标准的4例,缓解率86.67%;对照组达到2011年缓解标准的19例,未达到缓解标准的11例,缓解率63.33%。两组缓解率比较,差异具有统计学差异（P〈0.05）。结论：壮医热敏探穴针刺疗法结合壮药龙钻通痹方治疗类风湿关节炎临床疗效满意,无明显不良反应。Objective： To assess the efficacy of heat-sensitive acupuncture of Zhuang medicine combined with Longzuan Tongbi Decoction in patients with rheumatoid arthritis controlled with conventional therapy. Methods： A total of 64 patients with rheumatoid arthritis were randomly divided into the treatment group and the control group. The patients in control group received the conventional therapy based on the 2013 update of EULAR recommendations for the management of rheumatoid arthritis with synthetic and biological disease-modifying antirheumatic drugs,and the treatment group additionally received the heat-sensitive acupuncture of Zhuang medicine combined with Longzuan Tongbi Decoction. After a same period（ 4 weeks） therapy,the comparisons of several variables were analyzed. Results： Morning stiffness,grip strength,joint tenderness and joint swelling were significantly improved in the two groups,and the difference between the two groups was statistically significant（ P 〈 0. 01,P 〈 0. 05）.RF,CRP and ESR of the two groups were significantly reduced,and the difference of RF,CRP and ESR between the two groups was statistically significant（ P 〈 0. 01）. There were significant improvments in Pt GA,Ph GA and DAS28 of the two groups,but no significant difference were found（ P 〉 0. 05）. Conclusion： Heat-sensitive acupuncture of Zhuang medicine combined with Longzuan Tongbi Decoction could significantly reduce RF,CRP and ESR and improve morning stiffness,grip strength,joint tenderness,joint swelling and DAS28 of rheumatoid arthritis control with conventional therapy.
