摘 要:目的探讨血清中甲状旁腺激素(parathyroidhormone,PTH)和骨型碱性磷酸酶(bonealkalinephosphatase,BLAP)水平与骨质疏松骨折及临床预后的关系。方法选择2015年3月-2016年3月在本院健康体检的280例老年人群作为研究对象,均采用骨密度仪进行骨密度检测,并抽血检测患者血清中PTH、BLAP水平,采用Logistic回归分析探讨血清中PTH、BLAP水平与患者骨折发生的关系。结果 PTH和BLAP低水平患者腰椎、股骨颈、股骨粗隆、Ward三角区骨密度均低于对应高水平患者,差异有统计学意义(P<0.05);PTH、BLAP低水平组患者椎体压缩骨折、股骨颈骨折、骨盆骨折、其他骨折人数均高于对应高水平组患者,差异有统计学意义(P<0.05);Logistic回归分析结果提示,PTH(P=0.04)、BLAP(P=0.03)、股骨颈骨密度(P=0.01)是骨质疏松骨折的独立风险因素。结论血清中PTH、BLAP是患者骨质疏松发生的重要风险因素,对于老年患者监测BLAP、PTH水平可能有助于预测患者病情的发展。Objective To study the relationship of osteoporotic fracture and clinical prognosis in patients with parathyroid hormone(PTH)and bone alkalinephosphatase(BLAP)levels in serum of patients with fracture.Methods During March 2015-March 2016,280 elderly patients for health examination were collected from our hospital as the research objects,and bone density apparatus was adopted for bone density testing;blood samples were collected for detecting the serum levels of PTH and BLAP.Logistic regression analysis was used to investigate the relationship of serum PTH and BLAP levels with fractures.Results The bone densities of femoral neck,trochanter,ward triangle in patient with low level PTH and BLAP were lower than the high level patients,and the differences were statistically significant(P〈0.05);the number of patients with vertebral compression fractures,femoral neck fracture,pelvic fracture and other fractures in the group of low level PTH and BLAP was higher than that of the high level group,and the differences were statistically significant(P〈0.05).Logistic regression analysis showed that PTH(P = 0.04),BLAP(P = 0.03),femoral bone mineral density(P = 0.01)were independent risk factors for osteoporotic fracture.Conclusion PTH and BLAP in serum are important risk factors for the occurrence of osteoporosis in patients.Monitoring BLAP and PTH levels in elderly patients may help predict the development of patients' osteoporosis.
