摘 要:目的 探讨生物电阻抗(bioelectrical impedance analysis,BIA)体成分计测量住院老年人肌肉质量的重复性与复现性.方法 采用BIA测量50例老年患者的四肢肌肉质量.测量由两名经过统一培训的研究者进行,每名研究者测量两次,采用双因素方差分析判定影响重复测量差值的因素.采用重复性限、复现性限评价重复性或复现性条件下测量结果间差值的大小.组内相关系数(ICC)、组间标准差(Sw)、重复性系数(2.77Sw)评价两名操作者测量结果的可重复性;配对样本t检验、Bland-Altman法用于评价两名测一次测量结果的一致性.结果 方差分析显示,测量对象个体差异解释了总测量标准差的99%以上.测量结果的重复性限为0.64,复现性限为0.65.两名测量者各自测量结果间ICC〉0.99;Bland-Altman分析显示,两名测量者测量差值均数为0.02,95%一致限(95%LoA)为(-0.36,0.33),配对样本t检验显示两名测量者测量结果差异无统计学意义.结论 BIA对老年患者肌肉质量的测定具有良好的重复性和复现性.Objective To investigate the repeatability and reproducibility of a bioelectrical impedance analysis ( BIA ) consumer device in measuring of appendicular skeletal muscle mass of geriatric inpatients. Methods A total of 50 inpatients were measured by a BIA consumer device by two trained researchers. Each researcher measured two times randomly. Data of appendicular skeletal muscle mass were extracted. Double factor variance analysis was used to evaluate the factors influencing measurement difference. Repeatability and reproducibility limits were applied to evaluate the measurement difference in repeated and reproduced conditions. Intra-class correlation coefficient (ICC) , within-subject standard deviation (Sw) and repeatability coefficient (2.77Sw) were used to assess the repeatability of two research- ers. Paired t test and Bland-Altman analysis were used to assess the agreement between two researchers. Results ~ari- ance analysis showed individual variance in objects themselves accounted for over 99% overall standard devia- tion. Repeatability and reproducibility limits were 0. 64 and 0. 65 respectively. 1CCs were greater than 0. 99. Paired t test showed no statistical difference between two researchers and Bland-Altman analysis showed mean difference were 0. 02, 95% limits of agreements (LoA) were ( -0.36, 0.33) . Conclusion BIA device has high repeatability and repro- ducibility in measuring appendicular skeletal muscle mass of elderly inpatients.
