SuperPATH入路初次全髋关节置换术治疗髋部疾病的近期疗效分析

摘　　要：目的观察微创Super PATH入路初次单侧全髋关节置换术(THA)治疗髋部疾病的近期临床疗效。方法回顾性分析自2014-05—2015-08采用Super PATH入路的80例初次单侧THA,其中股骨头缺血性坏死45例,髋关节骨性关节炎32例,强制性脊柱炎2例,CroweⅡ型发育性髋关节发育不良1例。随访评估患者髋关节功能恢复情况,采用Likert量表调查患者满意度。结果 80例均获得2年随访。术后X线片显示髋臼假体固定良好且前倾满意,假体组件外展角为(41.03±7.25)°,双侧偏心距差值均≤4.2 mm,双下肢不等长差值均≤5.5 mm。末次随访时采用Likert量表调查患者满意度,72例很满意,8例良好。术后6周时72例可以独立行走、自行穿裤子、不需要服用止痛药。末次随访时79例未使用助行器,1例用助行器辅助行走。末次随访时髋关节功能Harris评分为96~99分,平均97.3分。结论 Super PATH入路THA治疗髋部疾病可较好地保留外旋肌群的完整性及功能,术中可将关节囊完整重建,术后髋关节假体初始稳定性好,具有创伤小、功能恢复快、并发症少、患者满意度高的优点,近期疗效满意。Objective To observe the short-term curative effect of SuperPATH approach for primary unilateral total hip replacement（THA）for the treatment of hip diseases.Methods A retrospective analysis of 80 cases of primary unilateral THA by using SuperPATH approach from May 2014 to August 2015 was conducted,including 45 cases of ischemic necrosis of the femoral head,32 cases of hip arthrophlogosis,2 cases of ankylosing spondylitis,and 1 ease of Crowe type II developmental dysplasia of the hip.The patients were followed up to evaluate the recovery of hip joint function,and the Likert scale was used to investigate the patient satisfaction.Results All 80 cases were followed up for 2 years.The postoperative X-ray showed that the acetabular prosthesis was well fixed and the anteversion was satisfactory.The abduction angle for prosthesis was（41.03±7.25）°,the difference of bilateral offset values were less than 4.2 ram,double limb length difference was less than 5.5 ram.In the last follow-up,the Likert scale was used to investigate the patient satisfaction.Seventy-two cases were satisfied,and 8 cases were good.At 6 weeks after the operation,72 cases were able to walk independently,wear their own pants by themselves,and did not need to take analgetic.In the last follow-up,79 cases were able to walk without aid and 1 case was assisted by aid.At the-nd of the follow-up,the Harris score of the hip joint function ranged from 96 to 99,with an average of 97.3.Conclusion Total hip arthroplasty with SuperPATH approach for the treatment of hip diseases can preserve the integrity and function of the external rotators,and it can reconstruct the joint capsule completely in operation.Owing to that,hip could obtain a good joint stability after surgery.Therefore,the advantages of SuperPATH approach are less trauma,faster function recovery with less complications and high patient satisfaction,significant short-term curative effect.
