摘 要:目的探究孕妇围生期维生素E摄入量、代谢与子痫前期(PE)的相关性,了解维生素E在PE发病过程中的作用。方法选取2012年6月-2016年12月该院收治的600例正常妊娠分娩孕妇(对照组)及500例PE孕妇(观察组)为研究对象,调查两组孕妇维生素E摄入情况,收集孕妇分娩时外周血,妊娠终止时采集胎盘、脐带、脐血及胎膜组织,对比两组孕妇维生素E水平的差异,分析两组孕妇胎膜、胎盘、脐带血SVCT1及SVCT2的表达以及SLC23al多态性。结果两组孕妇每日维生素E摄入量比较,差异无统计学意义(P>0.05);观察组孕妇外周血维生素E水平显著高于对照组,脐血维生素E水平显著低于对照组,差异均有统计学意义(P<0.05);观察组孕妇胎膜、胎盘、脐带血SVCT1及SVCT2的阳性表达率均显著低于对照组(P<0.05),两组孕妇rs33972313位点表现为G/G与A/G,rs34521685表现为A/A与A/G,rs35817838均表现为A/A,rs6886922表现为G/G与A/G,两组间各个位点表达分型比较,差异均无统计学意义(均P>0.05)。结论胎儿附属物SVCT1、SVCT2表达降低会影响母体与胎儿间维生素E的吸收与转运,使胎盘内抗氧化维生素E水平不足,可能导致PE的发病。Objective To explore the correlation between vitamin E intake and metabolism during perinatal period and preeclampsia in pregnant women,investigate the role of vitamin E in pathogenesis of preeclampsia. Methods From June 2012 to December 2016,600 normal pregnant women( control group) and 500 pregnant women with preeclampsia( observation group) treated in Shenzhen Maternity Child Healthcare Hospital were selected as research objects. Vitamin E intakes of pregnant women in the two groups were investigated. The peripheral blood samples of pregnant women in childbirth were collected. The placental,umbilical cord and umbilical cord blood,and fetal membrane tissue samples at the time of termination of pregnancy were collected. The differences of vitamin E levels between the two groups were compared. The expressions of SVCT1 and SVCT2 in fetal membrane,placental,and umbilical cord blood tissue samples of pregnant women and SLC23 al polymorphisms in the two groups were analyzed. Results There was no statistically significant difference in vitamin E intake between the two groups( P〈0. 05). The level of vitamin E in peripheral blood of pregnant women in observation group was statistically significantly higher than that in control group,while the level of vitamin E in umbilical cord blood of pregnant women in observation group was statistically significantly lower than that in control group( P〈0. 05). The positive expression rates of SVCT1 and SVCT2 in fetal membrane,placenta,and umbilical cord of pregnant women in observation group were statistically significantly lower than those in control group( P 0. 05). In the two groups,the site of rs33972313 expressed as G/G and A/G,the site of rs34521685 expressed as A/A and A/G,the site of rs35817838 expressed as A/A,the site of rs6886922 expressed as G/G and A/G.
