复元活血汤联合西药治疗脊柱骨折后腹胀、便秘(瘀热腑实)随机平行对照研究

摘　　要：[目的]观察复元活血汤联合西药治疗脊柱骨折后腹胀、便秘(瘀热腑实)疗效。[方法]使用随机平行对照方法,将90例住院患者按随机数字表方法随机分为两组。对照组45例头孢替安0.25g+250mL生理盐水,静滴,3次/d;乳酸钠林格500~1000mL/次,300~500mL/h,静滴;阿司匹林50~150mg/次,≤100mg/d;便秘严重使用开塞露。治疗组45例复元活血汤:桃仁、柴胡、天花粉、厚朴各15g,当归、枳壳、郁金、怀牛膝各10g,芒硝、酒大黄、红花、甘草各6g,纳差加炒麦芽、神曲各10g;疼痛剧烈加川楝子、延胡索各10g;恶心呕吐加竹茹、姜半夏各10g;尿闭尿少加木通、竹叶各10g;年老体弱可加芒硝、大黄各3g,火麻仁、肉苁蓉各15g。水煎300mL,1剂/d,3次/d,餐后30min温服;西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、疼痛评分、腹胀症状消失时间、便秘消失时间、不良反应。治疗1疗程(7d),判定疗效。[结果]治疗组显效24例,有效18例,无效3例,总有效率93.33%;对照组显效12例,有效19例,无效14例,总有效率68.98%;治疗组疗效优于对照组(P<0.01)。VAS评分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。腹胀、便秘消失时间治疗组短于对照组(P<0.01)。[结论]复元活血汤联合西药治疗脊柱骨折后腹胀、便秘(瘀热腑实),疗效满意,值得推广。[Objective] To observe Fuyuan Huoxue decoction combined with western medicine for the treatment of spinal fractures after abdominal distension and constipation. [Methods] 90 hospitalized patients were randomly divided into two groups according to the random digital table method, using random parallel control method. The control group of 45 cases of cefotiam 0.25g＋250mL saline, intravenous drip, 3 times/d; sodium lactate 500～1000 mL/, 300～500 mL/h, intravenous aspirin; 50～150mg/, less than 100 mg/d; the use of enema glycerini serious constipation. The treatment group of 45 cases of Fuyuan Huoxue decoction： peach, bupleurum, radix trichosanthis, Magnolia 15g, Angelica sinensis, fructus aurantii, radix curcumae, achyranthes 10g, rhubarb, mirabilite, safflower, licorice 6g, anorexia and malt and the divine comedy 10g; kagawa ko, the pain was severe nausea and vomiting with Corydalis 10g; Zhuru, Jiang Banxia 10g; add akebiaquinata, oliguria and anuria bamboo 10g; frail and Mangxiao, rhubarb 3g, fructus cannabis, cistanche 15g, decoction 300mL, 1 doses of/d, 3times/d, 30min after the meal.; westernmedicinewiththecontrolgroup. The continuous treatment of 7d was 1 courses. The clinical symptoms, pain scores, disappearance time of abdominal distension, disappearance time of constipation and adverse reactions were observed. The 1 course of treatment （7d） was treated and the curative effect was determined. [Results] The treatment group was markedly effective in 24 cases, effective in 18 cases, invalid in 3 cases, the total effective rate was 93.33%. In the control group, 12 cases were markedly effective, 19 cases were effective, 14 cases were invalid, the total effective rate was 68.98%, the treatment group was better than the control group （P〈0.01）. VAS scores were improved in two groups （P〈0.01）, and the improvement in the treatment group was better than that of the control group （P〈0.01）. The time of disappearance of abdominal distension and constipation was shorter in the treatment group
