莱菔子神阙穴外敷联合常规护理干预胸腰椎骨折术后胃肠道功能障碍随机平行对照研究

摘　　要：[目的]观察莱菔子神阙穴外敷联合常规护理干预胸腰椎骨折术后胃肠道功能效果。[方法]使用随机平行对照方法,将84例住院患者按随机数字表方法随机分为两组。对照组42例常规护理:去枕平卧,术后禁食禁水6h,75%酒精消毒伤口附近,3次/d,患肢抬高,促进血液循环,减轻肿胀,进食营养、易消化食物,适当帮助患者活动,防止压疮,术后24h指导功能锻炼,患肢踝背伸及股四头肌的等长收缩锻炼,收缩10s,放松10s,15~20次/组,3组/d,防止肌肉萎缩和关节僵硬。治疗组42例莱菔子置于5×5cm的5~6层纱布上,外敷神阙穴,10~15min/次,1次/d;常规护理同对照组。连续护理4d为1疗程。观测肠鸣音恢复时间、第1次排气时间、第1次排便时间、腹胀。治疗1疗程(4d),判定疗效。[结果]肠鸣音恢复时间护理组12h、24h人数明显多于优于对照组(P<0.05,P<0.01);第1次排气时间、第1次排便时间护理明显短于对照组(P<0.01);护理组腹胀6例(14.30%)低于对照组17例(40.50%)。[结论]莱菔子神阙穴外敷联合常规护理干预胸腰椎骨折术后胃肠道功能,能促进肠功能恢复,推荐深入研究。[Objective] To observe semen raphani Shenque acupoint application combined with routine nursing intervention of thoracolumbar fracture after operation of gastrointestinal function. [Methods] 84 hospitalized patients were randomly divided into two groups according to the random digital table method, using random parallel control method. The control group of 42 patients with routine care： to supine pillow, postoperative fasting 6 h, near the 75% alcohol disinfection of the wound, 3 times/d, limb elevation, promote blood circulation, reduce swelling, nutritious, digestible food, appropriate to help patients, prevent pressure sores, 24 h postoperative functional exercise guidance, isometric exercise limb, ankle dorsal extension unit four quadriceps contraction 10 s, relax 10 s, 15-20 times/group, 3 groups of/d, prevent muscle atrophy and joint stiffness. The treatment group of 42 cases in semen raphani 5×5 cm 5-6 layers of gauze, topical Shenque acupoint, 10-15 min/times, 1 times/d; control group with routine nursing care. Continuous nursing 4 d was 1 course of treatment. Observation of the recovery time of bowel sound, first times first times the exhaust time, defecation time, abdominal distension. The 1 course of treatment（4 d） was treated and the curative effect was determined. [Results] The time of borborygmus nursing group was significantly less than the number of 12 h and 24 h is better than that of control group（P〈0.05, P〈0.01）; first times first times the exhaust time, defecation time of nursing was significantly shorter than the control group（P〈0.01）; 6 cases of abdominal distension nursing group（14.30%） than in the control group of 17 cases（40.50%）. [Conclusion] Postoperative gastrointestinal function of semen Raphani Shenque acupoint application combined with routine nursing intervention of thoracolumbar fractures, can promote the recovery of intestinal function, recommended further research.
