摘　　要：目的探讨医院-社区-家庭一体化护理模式对脊髓损伤患者功能康复的影响。方法于2013年1月—2015年12月,将上海交通大学附属第六人民医院南院62例脊髓损伤出院患者按随机数字表法分为观察组30例和对照组32例,观察组实施医院-社区-家庭一体化护理模式干预1年,对照组按常规随访护理1年。于出院时、出院6个月及出院后1年3个时间点比较两组患者的Zung氏抑郁自评量表(self-rating depression scale,SDS)和焦虑自评量表(self-rating anxiety scale,SAS)评分、日常活动能力改良Barthel指数(modified barthel index,MBI)、压疮和尿路感染发生率、满意度等指标。结果观察组SDS得分、SAS得分及并发症发生率低于对照组,日常生活能力及满意度方面优于对照组,差异有统计学意义(P<0.05)。结论医院-社区-家庭一体化护理模式可为脊髓损伤患者提供全程、连续的护理服务,有助于重建患者的自信,提高其日常生活能力和生存质量。Objective To explore the effect of hospital-community-home integrated nursing model on functional recovery of patients with spinal cord injury. Methods From January 2013 to December 2015, 62 patients with spinal cord injury from South Branch of Shanghai Sixth People＇s Hospital, Shanghai Jiao Tong University were chosen. They were divided into the observation group （30 cases） and control group （32 cases） according to the random number table method. The observation group was implemented hospital-community-home integrated nursing mode for one year, while the control group was given routine follow-up care for one year. The scores of Self-Rating Depression Scale （SDS） and Self-Rating Anxiety Scale （ SAS）, modified Barthel index （ MBI）, incidence of pressure sores and urinary tract infections, satisfaction rate were analyzed. Results The observation group had lower scores of SDS and SAS, lower incidence of complications, higher MBI and higher satisfaction rate than the control group （P 〈 0.05 ）. Conclusion The hospital-community-home integrated nursing model for patients with spinal cord injury could provide whole course continuous nursing, rebuild their confidence, and effectively improve their daily living ability and quality of life.
