剖宫产术后用婴儿吸吮代替按摩按压子宫效果观察

摘　　要：目的比较剖宫产术后用婴儿吸吮代替常规的按摩按压宫底排积血的护理措施,及其对产后出血及母亲疼痛的影响。方法剖宫产术病人480例,随机分为2组,试验组在剖宫产产后每日评估时不再常规的按摩子宫而改让宝宝吸吮。对照组按常规操作,产后每日评估时常规人工按摩子宫排血。比较2组产后出血率和产妇疼痛反应等指标。结果按产后出血大于500 m L和大于1 000 m L 2个层次统计产后出血的发生率,2组在产后出血发生率比较无统计学意义(P>0.05);产后评估子宫收缩情况时,对照组有更多的重度疼痛患者,试验组多数为轻度和中度疼痛。结论剖宫产术后不常规按摩子宫,而是利用婴儿的吸吮来代替,刺激引发宫缩,评估产后出血量,是可行安全的方法,可明显减轻产妇的疼痛,并且没有增加产后出血量及产后出血的发生率。Objective Comparisons of the clinical effect of replacement of routine uterine massage with baby sucking of postpartum hemorrhage and severe of pain for post cesarean section patient.Methods 480 cases of post cesarean section patients were randomized to two groups,the experimental group were adapted to baby sucking care protocol in daily routine nursing assessment,control group were received routine uterine massage care protocol.Postpartum hemorrhage and pain of the mothers were recorded.Results there were no statistical significant in the volume and rate of the postpartum hemorrhage either over than 500 m L and over 1000 m L subgroups（P〈0.05）.the controlled group had more severe pain when routine assessment using uterine massage.Experimental groups had more moderate and minor pain.Conclusion It is a safe and feasible nursing care protocol for post cesarean section patient to adapted to baby sucking in replacement of routine uterine massage,resulted in less pain for the mothers,no increasing of the volume and the rate of postpartum hemorrhage.
