老年心血管病并发肺炎人群临床特点与病原菌的耐药性探讨

摘　　要：目的通过对老年心血管病并发肺炎人群临床特点与病原菌的耐药性分析,为临床合理性用药及控制感染提供依据。方法探讨老年心血管病并发肺炎人群临床资料与病原菌的耐药性。结果老年心血管病并发肺炎患者的临床特点其多数患者均患有基础类疾病;1632例老年心血管病患者中有218例并发肺炎,感染发生率为13.4%,分离病原菌293株,主要为革兰阴性杆菌224株(76.4%),以大肠埃希菌、肺炎克雷伯杆菌、铜绿假单胞菌、鲍氏不动杆菌为主;革兰阳性球菌40株(13.7%),以金黄色葡萄球菌、肠球菌属为主;真菌29株(9.9%)以白色假丝酵母菌为主,常见病原菌耐药性均较高。结论老年心血管病并发肺炎人群常见病原菌耐药性均较高,临床上需要根据药敏试验结果来进行合理性用药。Objective To analyze the clinical characteristics and drug resistance of pathogens in elderly patients with cardiovascular disease complicated by pneumonia, so as to provide scientific basis for rational clinical use of drug and the prevention of infection. Methods To discuss the clinical data and drug resistance in elderly patients with cardiovascular disease complicated by pneumonia. Results The clinical characters of elderly patients with cardiovascular disease complicated by pneumonia were normally with basic diseases; among ! 632 cases of elderly patients with cardiovascular disease complicated by pneumonia, 218 cases were found to be complicated with pneumonia（ 13.4% ）, and 293 pathogens were isolated. Common pathogens were 224 Gram negative bacillus （76.4%） （ mainly were Escherichia coli, KlebsieUa pneumoniae, Pseudomonas aeruginosa, Acinetobacter baumannii and so on）, 40 strains were Gram positive cocci（ 13.7% ） （ mainly were Staphylococcus aureus, Enterococcus）, 29 strains were funguses（9.9% ） （mainly were Candida albicarts）. The common pathogens were highly drugresistant. Conclusion The clinical characters of elderly patients with cardiovascular disease are that its common pathogens are highly resistant, rational clinical drug use should be based on drug sensitive test results.
