摘 要:目的利用基于图论的分析方法,研究原发性闭角型青光眼(PACG)患者静息态下脑网络的小世界属性。方法 33例PACG患者及33例性别、年龄匹配的健康对照组(HCs),均接受静息态f MRI检查。选择116个自动解剖标定(AAL)模板脑区为节点,计算功能连接获得相关矩阵用于构建二值化网络,获取脑网络整体及局部属性,并在局部属性上行组间双样本t检验(错误发现率矫正,P〈0.05)分析差异脑区,同时分析差异脑区的网络属性与临床参数如视网膜纤维层厚度(RNFLT),静脉杯盘比(CDR),眼内压(IOP)及视力(VA)的相关性。结果 PACG及HCs组脑网络均存在"小世界"属性。在最小及最大网络稀疏度(0.32-0.42)下,PACG组聚类系数以及小世界指数均轻度减低,但差异无统计学意义(P〉0.05)。在最小网络稀疏度0.32下,PACG患者视觉及非视觉(躯体运动及情感-记忆)脑区节点度及节点介数有改变,且其左侧眶部额中回节点介数与静脉CDR呈正相关,双侧缘上回及左侧豆状壳核节点介数与VA呈正相关,右侧楔叶节点度与VA呈负相关。结论 PACG患者脑网络存在小世界属性,其视觉、躯体运动及情感-记忆脑区局部属性存在改变,表明PACG患者脑网络传递信息效率异常,为理解PACG全脑网络功能改变提供新思路。Objective To study the small-world properties of brain networks in primary angle-closure glaucoma( PACG) patients in resting state using graph-based analysis. Methods Thirty-three patients with PAG and 33 healthy controls( HCs) with age-matched controls received resting-state f MRI. 116 automated anatomical labeling( AAL) template brain regions were selected as nodes,and the functional connectivity was obtained to obtain the correlation matrix for constructing the binarization network,and the brain network overall and local attributes were obtained,and two samples were performed between the local attribute uplink groups. T-test( false discovery rate correction,P〈0. 05) was used to analyze the differences in brain regions. Network properties and clinical parameters such as retinal fibrous layer thickness( RNFLT),CDR,intraocular pressure( IOP) were analyzed at the same time.) and visual acuity( VA) correlation. Results There were "small-world"attributes in the brain networks of PACG and HCs groups. Under the minimum and maximum network sparsity( 0. 32 - 0. 42),PACG group clustering coefficient and small-world index all decreased slightly,but the difference was not statistically significant( P〈0. 05). Under the minimum network sparsity of 0. 32,the degree of node and node in the brain area of visual and non-visual( physical and emotional-memory) changes in PACG patients were positively correlated with the venous CDR. There was a positive correlation between the nucleus and VA of the bilateral marginal gyrus and the left dorsal nucleus,and there was a negative correlation between the VA and the node degree of the right wedge. Conclusion The brain network of PACG patients has a small-world property,and there are changes in the visual,somatic movement,and local attributes of the emotional-memory brain regions.
