摘　　要：目的总结透明细胞型脑膜瘤的MRI影像特征,提高对该病MR影像表现的认识。方法回顾分析经病理证实且术前被误诊为非脑膜瘤的25例透明细胞型脑膜瘤患者的MR影像表现及临床资料。结果 17例女性,8例男性,发病中位年龄为29岁。8例位于桥小脑区,5例位于大脑凸面,5例位于静脉窦旁,3例位于鞍旁,2例位于前颅窝底中线,2例沿硬脑膜或软脑膜播散表现为多发肿块。透明细胞型脑膜瘤表现为实性肿块20例,多呈T1WI等信号、T2WI等或低信号影。肿瘤表现为囊实性肿块2例,实性部分呈＂石榴＂状,T2WI呈低信号、T1WI呈等信号影,增强扫描可见实性部分明显强化。肿瘤表现为囊性肿块3例,呈T1WI低信号、T2WI高信号,而增强扫描成＂丝瓜瓤＂样轻度不均匀强化。结论透明细胞型脑膜瘤不具有典型脑膜瘤的影像学表现,但其发病部位及影像表现有自身特征性表现。Objective To summarize the MRI features of clear cell meningioma,and to improve the understanding of MR image. Methods Retrospectively analyzed 25 cases of clear cell meningioma which were pathologically proved with the MR findings and clinical data and misdiagnosed as non-meningioma. Results there were 17 females and 8 males with a median age of 29 years. 8 cases were located in the cerebellopontine angle,5 cases located in the convexity of brain,5 cases located adjacent to the the venous sinus. 3 cases located in the parasellar region,2 cases located in the midline of anterior cranial fossa and 2 cases showed multiple mass spread along with the pachymeningeal or leptomeningeal. Clear cell meningioma showed solid mass in 20 cases with isointense on T1WI and iso/hypointense on T2WI. The tumor showed cystic and solid mass in 2 cases. The solid part seems like＂pomegranate＂with hypointense on T2WI and isointense on T1WI. After contrast,the solid part showed obvious enhancement. The tumor showed cystic mass in 3 cases with hypointense on T1 WI and hyperintense on T2WI and＂loofah＂slightly heterogeneous in the enhancement. Conclusion clear cell meningioma has no typical imaging features of meningioma,but its location and imaging findings have their own characteristics.
