摘 要:目的探究IL-6水平是否能作为慢性肾脏病(Chronic kidney disease,CKD)患者预后的预测因子。方法本实验是一项前瞻性队列研究,选择245例2~4期CKD患者(根据MDRD研究方程算得e GFRs在15~90 m L/min·1.73m2)。检测其血清IL-6浓度。随访时间为6年,随访的主要终点是急性心血管事件,次要终点为肌酐倍增的发生。采用Kaplan Meier模型来验证基线IL-6水平对慢性肾脏病患者发生急性心血管事件及肌酐倍增的影响。结果研究对象平均年龄为60岁。44%是男性。发生心血管事件50例,49例患者肌酐倍增。IL-6水平最高四分位组(>3.2 pg/m L)发生终点事件的概率比IL-6最低四分位组(<1.2 pg/m L)、第二四分位组(1.2~1.9 pg/m L),第三四分位组(1.9~3.2 pg/m L)都高。结论基线IL-6水平可成为慢性肾脏病患者预后的预测因子。Objective To investigate if IL-6 level could be used to evaluate the prognosis of chronic kidney disease.Methods This study was a prospective study,244 patients with stage CKD 2-4(according to the MDRD equation for EGFRs at 15-90mL/min· 1.73m2)were selected and serum IL-6 concentration was measured.The follow-up time was six years.The primary endpoint was the occurrence of acute cardiovascular events,and the secondary endpoint was the occurrence of doubled creatinine.Kaplan Meier model was used to examine the relationship between baseline IL-6 levels and acute cardiovascular events in patients with chronic kidney disease.Results The mean age of the subjects was 60 years old.44% were men.50 cases were having cardiovascular events,49 patients had doubled creatinine.The risk rate of end point in the highest IL-6 group( 〉 3.2 pg / mL)was the highest among all the groups.Among them,the risk rate of end point in the second four quantile group was 1.2~1.9 pg / mL,that in the third four quantile group was 1.9~3.2pg / mL and risk rate in the lowest four quantile group was lower than 1.2 pg / mL.Conclusions Elevated baseline IL-6 levels are associated with an increased risk of acute cardiovascular events and doubled creatinine in patients with chronic kidney disease.
