摘　　要：目的：研究与分析切开复位锁定板内固定术与半肩关节置换术治疗老年肱骨近端骨折的临床效果。方法：选取我院2014年-2017年收治的老年肱骨近端骨折患者90例，根据治疗方式的不同分为观察组（18例）和对照组（72例）。前者采用半肩关节置换术治疗，后者采用切开复位锁定板内固定术治疗。比较2组治疗效果。结果：经治疗，观察组术中出血量、手术时间、Neer评分以及治疗优良率等，均显著优于对照组，且P〈0．05，差异具有统计学意义。结论：针对老年肱骨近端骨折，采用半肩关节置换术治疗的效果显著，能显著改善患者术后功能。Objectiv： To study the clinical effect of open reduction and locking plate internal fixation and hemiarthroplasty in treatment of senile proximal humeral fractures. Methods： in our hospital from 2014 to 2017 were elderly patients with proximal humeral fractures in 90 cases, according to the different treatment for replacement group （ 18 cases） and the internal fixation group （72 cases）. The former adopts semi shoulder joint replacement therapy, the latter by open reduction and locking plate fixation. The treatment effect of the two groups were compared. Results： after treatment, the amount of bleeding and replacement in operative time, Neer score and the excellent rate of trcatment, were significantly better than internal fixation group. And The difference of P 〈 0.05 was statistically significant. Conclusion ： the effect of hemiarthroplasty in the treatment of proximal humeral fractures in elderly patients is remarkable and can improve the postoperative function of the patients.
