摘　　要：目的探讨伴稳定型冠状动脉粥样硬化性心脏病(冠心病)的慢性牙周炎患者进行全程心电监护下牙周机械治疗的安全性。方法选取2013年4月至2015年5月在首都医科大学附属北京安贞医院口腔科就诊的伴稳定型冠心病的慢性牙周炎患者69例作为观察组;同期选取单纯慢性牙周炎患者50例作为对照组。2组患者均在全程心电监护下进行牙周机械治疗(包括牙周探诊、龈上洁治和龈下刮治),观察并记录患者的心率、血压、血氧饱和度变化。结果 2组患者均顺利完成了牙周机械治疗,术中及术后的心率、血压及血氧饱和度无异常波动。观察组在牙周洁治后5 min时的收缩压明显高于对照组[(124±10)mmHg(l mmHg=0.133 kPa)比(120±10)mmHg](P=0.035),其余术中、术后各时点2组收缩压、舒张压、心率、血氧饱和度差异均无统计学意义(均P>0.05)。结论对伴稳定型冠心病的慢性牙周炎患者可以安全地进行牙周机械治疗。Objective To investigate the safety of periodontal mechanical therapy with whole course electrocardiographic monitoring in chronic periodontitis patients with stable coronary heart disease. Methods From April 2013 to May 2015, 69 chronic periodontitis patients with stable coronary heart disease were enrolled as observation group and 50 patients with simple chronic periodontitis were enrolled as control group in Beijing Anzhen Hospital, Capital Medical University. All patients had periodontal mechanical treatments （probing, supragingival and subgingival scaling） with whole course electrocardiographic monitoring. Changes of heart rate, blood pressure and oxygen saturation were recorded. Results No abnormal fluctuations of heart rate, blood pressure and oxygen saturation were observed during and after periodontal treatments. Systolic pressure 5 mih after supragingival scaling in observation group was significantly higher＇than that in control group[ （124 ± 10）mmHg vs （120 ± 10）mmHg] （P = 0. 035 ）. Systolic pressure, diastolic pressure, heart rate and oxygen saturation at any other time-points during and after sealing had no significant differences between groups （ P 〉 0. 05 ）. Conclusion Periodontal mechanical therapy can be safely performed in stable coronary heart disease patients.
