度冷丁用于高强度超声聚焦治疗子宫腺肌病患者镇痛效果及护理观察

摘　　要：目的探讨度冷丁用于高强度超声聚焦治疗子宫腺肌病患者镇痛效果及护理观察。方法将我院收治的80例高强度超声聚焦治疗子宫腺肌病患者作为研究对象,随机分为两组。静脉全麻组40例采用静脉全身麻醉(咪达唑仑、芬太尼、丙泊酚),度冷丁肌注组40例采用度冷丁肌肉注射辅助麻醉。在治疗过程中给予全程优质护理措施。比较两组患者MAP、HR水平、麻醉相关指标的差异,并分析其麻醉不良反应。结果两组患者在麻醉诱导前、苏醒时MAP、HR水平比较,差异无统计学意义(P>0.05);静脉全麻组患者超声聚焦时较麻醉诱导前MAP水平显著降低,HR水平显著增高,差异有统计学意义(P<0.05);度冷丁肌注组患者超声聚焦时较麻醉诱导前MAP、HR水平无明显变化,差异无统计学意义(P>0.05);静脉全麻组患者苏醒时间、定向力恢复时间、意识清楚时间显著高于对照组,差异有统计学意义(P<0.05);静脉全麻组患者不良反应发生率为40.0%高于度冷丁肌注组5.0%,差异有统计学意义(P<0.05)。结论高强度超声聚焦治疗子宫腺肌病应用度冷丁肌肉注射麻醉效果好、血流动力学影响小、不良反应少,值得临床选择。Objective To observe the analgesic effect and nursing observation of dolantin in patients with adenomyosis treated by high intensity focused ultrasound. Methods Eighty adenomyosis patients treated by high intensity focused ultrasound（HIFU） in our hospital were selected and randomly divided into two groups,40 cases in each group.The intravenous anesthesia group adopted the general intravenous anesthesia（midazolam,fentanyl,propo fol）;Dolantin group adopted the auxiliary anesthesia of dolantin and the high quality nursing interventions.The index of MAP,HR and related anesthesia index and the adverse reactions of two groups were compared. Results Before anesthesia induction and after awakening,the index of MAP and HR of two groups were not significantly different（P 0.05）;The MAP level of intravenous anesthesia group during the HIFU was significantly lower than before anesthesia induction,the HR level was greatly increased,there were statistical differences（P 0.05）;The MAP and HR levels of dolantin group during the HIFU and before anesthesia induction were not significantly different, there no statistical differences（P 0.05）;The awakening time,orientation recovery time and consciousness time of intravenous anesthesia group were significantly longer than those of dolantin group（P 0.05）;The adverse event rate of intravenous anesthesia group（40.0%） was significantly higher than that of dolantin group which was 5.0%（P0.05）. Conclusion In the treatment of adenomyosis with high intensity ultrasound,the dolantin has a better anesthesia effect, the influence of hemodynamics is small, and the adverse reaction is less.It is worthy of clinical selection.
