摘　　要：目的通过将Narcotrend监测用于接受加速康复外科(ERAS)的老年肝胆手术患者,观察其对术后恢复和住院费用及时间的影响。方法将60例患者随机分为2组.对照组(A组)根据经验麻醉用药,观察组(B组)采用Narcotrend监测来指导麻醉用药,观察患者苏醒时间及在恢复室驻留时间,术后并发症发生情况和住院费用和时间。结果与A组相比,B组患者术后苏醒时间和恢复室驻留时间更短(P<0.05),术后恶心呕吐(PONV)及肺部感染发生率更低(P<0.05),住院费用及时间更低(P<0.05),而术后认知功能障碍(POCD)和谵妄的发生率两组比较无统计学意义。结论将Narcotrend监测用于接受ERAS的老年肝胆手术患者,可降低PONV及术后肺部感染发生率,缩短住院时间,减少住院费用,有利于术后快速康复,可成为老年肝胆手术患者ERAS的一部分。Objective To observe the effect on recovery after surgery and the cost and days in-patient by the use of monitor of Narcotrend for the elder underwent Liver-gallbladder surgery with enhanced recovery after surgery（ERAS）. Methods The 60 patients were divided into two groups randomly. Control group（group A） administer anesthesia agent based on experience while observation group（group B） based on the monitor of Narcotrend,then observe the time of patients of recovery and stay in Postanesthesia Care Unit（PACU）,the complications after surgery and the cost and days in-patient. Results Contrast to group A,the time of recovery and stay in PACU of the patients in group B is shorter（P0.05）,the incidence of the Postoperative Nausea and Vomit（PONV） and the lung infection is lower（P0.05）,meanwhile,the cost and length of hospitalization is lower（P0.05）,although the incidence of the Postoperative Cognitive Dysfunction（POCD） and delirium have no statistical significance between two groups.Conclusion It can reduce the incidence of PONV and lung infection postoperative to use the monitor of Narcotrend for the elder underwent Liver-gallbladder surgery with ERAS,and shorten the length of hospitalization and reduce the cost,which is beneficial to the quick recovery after operation,therefore it can be a part of ERAS in elderly patients underwent liver and bile surgery.
