摘 要:评估中国2型糖尿病早诊早治的健康经济结果。应用英国糖尿病前瞻性研究模型模拟健康结果及并发症治疗成本;并进行成本效用分析。在5年的模拟中,干预组并发症治疗成本6 186.30元,对照组7 183.91元;两组的预期寿命均为4.7年,质量调整生命年均为3.7QALYs。在30年的模拟中,干预组并发症治疗成本31 011.92元,提前5年的降糖成本6 022.5元,预期寿命为10.5年,质量调整生命年8.0 QALYs;对照组并发症治疗成本33 010.98元,预期寿命为10.4年,质量调整生命年7.9 QALYs;30年模拟中两组的增量成本效果比为40 234.4元/QALY。早诊早治有高度的成本效果,应加强糖尿病的早诊早治。To estimate the economic impacts and health consequences of early treatment for type-2 diabetes in China. UKPDS model was applied to simulate the health consequences and medical costs associated with diabetic complications. And cost-utility analysis was applied to the results. In 5-years simulation,medical costs of diabetic complications are 6 186. 30 $ for the intervention group,and 7 183. 91 $ for the control group; Life expectancy and quality-adjusted life years for both groups are 4. 7 years and 3.7 QALYs. For the intervention group in 30-years simulation,medical costs of diabetic complications are 31 011. 92 $,medical expenditures of 5-years earlier glycaemia intervention are 6 022. 50 $,life expectancy are 10. 5 years,and quality-adjusted life years are 8. 0 QALYs; for the control group in 30-years simulation,medical costs of diabetic complications are 33 010. 98 $,life expectancy are 10. 4 years,and quality-adjusted life years are 7. 9 QALYs; ICER of the two groups in 30 years is 40 234. 4 $/QALY. Early treatment of type-2 diabetes is highly cost-effective,and hence should be promoted.
