颅内动脉瘤介入治疗手术麻醉中丙泊酚及舒芬太尼联合应用效果研究  

The Effect of Sufentanil Compound Propofol Applicated in Interventional Operation of Intracranial Aneurysms

摘　　要：探讨丙泊酚复合舒芬太尼在动脉瘤介入手术麻醉中的临床应用效果,选取具有动脉瘤介入手术指征的患者200例,分为舒芬太尼组及丙泊酚复合舒芬太尼组,比较两组血流动力学变化、麻醉恢复情况、恶心、呕吐、躁动等不良反应发生率。结果显示:丙泊酚复合舒芬太尼组在插管即刻、插管后1min、插管后5min、拔管时的收缩压、舒张压、心率均显著低于舒芬太尼组(P<0.05);自主呼吸恢复时间、呼之睁眼时间、拔管时间、定向力恢复时间均短于舒芬太尼组(P<0.05)。因此,丙泊酚复合舒芬太尼在动脉瘤介入手术的麻醉中使血流动力学更平稳,苏醒质量更高,不良反应率降低。200 cases of patients with aneurysms were treated with intervention operation. According to random number table method and all cases were divided into sufentanil intravenous anesthesia group and propofol compound sufentanil intravenous anesthesia group. The authors monitored the systolic blood pressure,diastolic blood pressure,heart rate,by Datex Ohmeda at the different time point,before intubation（ T_0）,immediate intubation（ T_1）,1 min after intubation（ T_2）,5 min after intubation（ T_3）,tube drawing（ T_4）. The authors compared the hemodynamic changes,anesthesia recovery including spontaneous breathing recovery time,called the open time,extubation time,directional force and the incidence of adverse reactions such as nausea,vomiting,restlessness between two groups. The systolic blood pressure,diastolic blood pressure,mean arterial blood pressure,heart rate,blood oxygen saturation and end-tidal CO_2 partial pressure of patients in propofol compound sufentanil group at T1,T_2,T_3,T_4 were significantly lower than those of sufentanil group（ P〈0. 05）. The spontaneous breathing recovery time,called the open time,extubation time and orientation recovery time of patients in propofol compound sufentanil group of were significantly shorter than those of sufentanil group（ P〈0. 05）. Patients in propofol compound sufentanil group had the smaller possibility to appeared nausea,vomiting,agitation,syncope cases than sufentanil group. Compared to sufentanil,propofol compound sufentanil used in aneurysm operation makes more stable blood stream dynamics,higher quality of vivification and fewer adverse reaction rate.
