摘 要:目的评价生物电阻抗无创心排血量监测(electrical velocimetry,EV)在先天性心脏病患儿术后血流动力学应用的效果及临床意义。方法选取2016年7月至2016年11月于河北医科大学第三医院住院治疗的30例先天性心脏病患儿,均在体外循环下行先天性心脏病根治手术。患儿分别于术前及术后12、24、48h应用EV和经胸超声心动图(transthoracic echocardiography,TTE)测量记录心排血量指数(cardiac index,CI)、每搏输出量指数(stroke volume index,SVI)数值。应用Altman-Bland法检验两种方法检测结果的一致性。结果患儿术后12、24、48h的CI、SVI比较,差异无统计学意义(P>0.05)。EV法测的CI及SVI与传统TTE测量的CI及SVI之间存在较好的一致性。结论 EV应用于先天性心脏病患儿术后血流动力学检测准确有效。Objectives To investigate the clinical applicability of electrical velocimetry(EV)in monitoring pediatriccardiac output after cardiopulmonary bypass. Methods From July 2016 to November 2016,30 children with congeni-tal heart disease underwent cardiac operation in The Third Hospital of Hebei Medical University were selected. Cardiacindex(CI),stroke volume index(SVI)of children before cardiac surgery and 12h,24h,48h after cardiac surgerywere measured by EV and transthoracic echocardiography(TTE). Consistency of the results of the two methods wastested by Altman-Bland method. Results There were no significant differences of CI,SVI tested by EV and TTE inchildren 12h,24h,48h after cardiac surgery. EV and traditional TTE method had a good consistency in measuringCI and SVI values. Conclusions EV could accurately and effectively monitor the degree of postoperative hemodynamicin children after extracorporeal circulation.
