摘　　要：目的了解简单先天性心脏病(先心病)患者疾病经济负担及影响因素。方法采用时间段连续病例整群抽样法,对2012年至2013年在广东省某三甲医院心外科住院的184例先心病患者和家长进行流行病学调查,并同时搜集患者的住院病历资料。结果房间隔缺损和室间隔缺损患者疾病总经济负担中位数为39421元,其中直接经济负担中位数为31254元,间接经济负担中位数为1400元。先心病经济负担影响因素统计结果显示住院时间、在复苏室时间、有无导管检查、先心病诊断类型、年龄的OR值和95%CI分别为1.202(1.1~1.315)、1.551(1.12~2.149)、4.402(1.629~11.882)、0.152(0.07~0.33)和7.925(2.532~24.779)。结论常见简单先心病患者经济负担以直接经济负担为主,其中住院时间、在复苏室时间、是否做导管、先心病诊断类型和患者年龄对先心病患者的直接经济负担有影响,提示今后应加强产前检查工作和优生优育教育力度,以有效降低先心病的发生,减低先心病经济负担。Objectives To understand the factors affecting economic burden in patients with simple congenital heartdiseases. Methods An epidemiological survey was carried out in patients（184 cases）with congenital heart diseasein a hospital in Guangdong Province from 2012 to 2013,and the data in hospitalized medical records were alsocollected at the same time. Results Median economic burden of patients with simple congenital heart diseases was39421 yuan,of which the median of direct economic burden was 31 254 yuan,and the median of indirect economicburden was 1400 yuan. The results showed that hospitalization-duration,resuscitation duration,yes or no catheter-ization,type of congenital heart disease and patient′s age affected economic burden,the OR（95% CI）was 1.202（1.1-1.315）,1.551（1.12-2.149）,4.402（1.629-11.882）,0.152（0.07-0.33）and 7.925（2.532-24.779）,respectively.Conclusions Economic burden of simple congenital heart diseases is mainly affected by direct economic burden.Hospitalization-duration,resuscitation duration,type of congenital heart disease,yes or no catheterization andpatient age have impacts on direct economic burden of congenital heart disease,which suggesting that the futureprenatal examination and eugenics education would be taken so as to effectively reduce the incidence of congenitalheart disease.
