摘 要:目的探讨脑肿瘤患者认知异常的静息态脑功能成像。方法选择19例脑肿瘤患者为脑肿瘤组,23例健康体检的志愿者为对照组。所有受试者在静息状态下完成血氧水平依赖功能磁共振成像(BOLD-f MRI)检查,采用静息态脑功能成像方法对静息态功能磁共振成像(rs-f MRI)数据进行局部一致性影像(Re Ho)、低频振幅影像(ALFF)处理,通过RESTPLUS软件分析脑肿瘤组与对照组Re Ho值、ALFF值的差异。结果与对照组比较,脑肿瘤组双侧额下回、双侧颞中回、双侧海马旁回、右侧前扣带回ALFF值均明显升高(均P<0.01);中脑、左侧颞中回、双侧颞上回、双侧海马旁回、双侧前扣带回、双侧顶上回、双侧额下回、双侧辅助运动区Re Ho值均明显升高(均P<0.01)。结论脑肿瘤患者在额叶、颞叶、边缘系统及辅助运动区存在着一系列脑部自发神经活动的变化,激活的区域与目前对该脑区认知功能的研究基本符合,且以双侧大脑半球改变为多。Objective To analyzed the resting state brain imaging of cognitive abnormalities in patients with intracranial tumors. Methods Nineteen patients with intracranial tumors and 23 healthy individuals(control group) underwent complete BOLD-fMRI examination in resting state. The BOLD data were post-processed by RESTPLUS to derive the differences in regional homogeneity(ReHo) and amplitude of low-frequency fluctuation(ALFF) value between brain tumor patients and control group. Results Compared with control group, the brain tumor group showed a set of increasing value of ALFF in the bilateral inferior frontal and middle temporal gyrus,a nd limbic system like right anterior cingulate gyrus and bilateral parahippocampal gyrus(P〈0.01). Compared with the control group, the brain tumor group presented the increased value of ReHo in the midbrain,bilateral inferior parietal, inferior frontal, left middle temporal, and limbic system like bilateral parahippocampal gyrus and anterior cingulate gyrus, as well as bilateral supplementary motor area(P〈0.01). Conclusion Patients with intracranial tumor have a series of changes in the frontal, temporal, limbic system and supplementary motor areas; and most of them are bilateral cerebral hemisphere-changed. The activated areas are basically consistent with the results from the functional cognitive study.
