交错内痔套扎、弧形切扎外痔保留齿线术治疗环状混合痔的临床研究

摘　　要：目的:探讨交错内痔套扎、弧形切扎外痔保留齿线术在环状混合痔患者治疗中的应用效果。方法:选取某院2015年1月~2017年1月间收治的环状混合痔患者50例作为研究对象,采用乱数表法均分为对照组和研究组,每组各25例。对照组患者采用传统的外剥内扎术进行治疗,研究组患者则采用交错内痔套扎、弧形切扎外痔保留齿线术进行治疗,比较两组治疗总有效率、并发症发生情况、术后疼痛评分以及创面愈合时间。结果:两组患者治疗总有效率、并发症发生情况无统计学差异,对照组患者术后6h疼痛评分明显高于研究组(t=8.371,P<0.05);术后1d和7d,两组患者疼痛评分无统计学差异(t=1.136,0.851,P>0.05);对照组患者平均愈合时间高于研究组,差异显著(t=4.537,P<0.05)。结论:环状混合痔患者采用交错内痔套扎、弧形切扎外痔保留齿线术治疗,在治疗有效率和并发症发生情况方面与传统外剥内扎术无差异,但可显著降低患者术后疼痛评分,缩短创面愈合时间,有助于减轻患者的痛苦,值得在临床推广应用。Objective：To investigate the application effect of interlaced internal hemorrhoids ligation,arcuate incision of external hemorrhoids to preserve tooth line in the treatment of annular mixed hemorrhoids.Methods：50 patients with annular mixed hemorrhoids treated in a hospital from January 2015 to January 2017 were selected as the research object and divided into control group and study group using random table method,with 25 cases in each group.The control group were treated with traditional external exfoliation and internal ligation,and the study group were treated with interlaced internal hemorrhoids ligation,arcuate incision of external hemorrhoids to preserve tooth line,then the total efficiency of treatment,complications,wound healing time and postoperative pain score in the two groups were compared.Results：There was no statistical difference between the two groups in the total effective rate of treatment and the incidence of complications.The pain score in the control group was significantly higher than that in the study group at 6 hours postoperatively（t=8.371,P〈0.05）.There was no significant difference in pain scores between the two groups on the 1 st and 7 th day after operation（t=1.136,0.851,P〉0.05）.The average healing time in the control group was higher than that in the study group,with significant difference（t=4.537,P〈0.05）.Conclusion：There is no difference in the effective rate of treatment and the incidence of complications for patients with annular mixed hemorrhoids treated with the traditional external exfoliation and internal ligation or treated with interlaced internal hemorrhoids ligation,arcuate incision of external hemorrhoids to preserve tooth line,but the application of interlaced internal hemorrhoids ligation,arcuate incision of external hemorrhoids to preserve tooth line can significantly reduce the postoperative pain score and shorten the wound healing time,which is helpful to alleviate the pain of the patients,and is worthy of clinical promotion and application.
