摘 要:目的分析正常早孕期和不明原因自然流产妇女蜕膜组织中CXCR6^+单核-巨噬细胞比例及表型差异。方法收集上海复旦大学附属妇产科医院门诊手术室正常早孕期(40例)和不明原因自然流产(20例)妇女的蜕膜组织,分离蜕膜免疫细胞群,采用流式细胞术分析正常早孕组及不明原因自然流产组蜕膜免疫细胞中单核-巨噬细胞所占比例的差异;比较两组妇女蜕膜单核-巨噬细胞(dMΦ)表面CXCR6^+、CD80、CD206的表达差异。结果不明原因自然流产组CD14+dMΦ在蜕膜免疫细胞群(DIC)中所占比例为(9.9±1.63)%,正常早孕期组CD14+dMΦ在DIC中所占比例为(8.9±0.88)%,两组差异并无显著性(P>0.05);正常早孕组CXCR6^+在dMΦ的表达率为(40.0±1.90)%,而不明原因自然流产组表达率为(31.22±2.38)%,二者比较有显著差异(P<0.05);正常早孕组CD206、CD80在CXCR6^+dMΦ中的表达均显著高于CXCR6^-dMΦ(P<0.05),不明原因自然流产组CD206在CXCR6^+dMΦ的表达显著高于CXCR6^-dMΦ(P<0.01),CD80并无显著变化(P>0.05);与正常早孕组相比,不明原因自然流产组CD206在CXCR6^(+/-)dMΦ中的表达均显著降低(P<0.05),CD80在CXCR6^-dMΦ中表达则显著升高(P<0.05),在CXCR6^+dMΦ中的表达无显著差异(P>0.05)。结论不明原因自然流产妇女中,蜕膜单核巨噬细胞数量并无显著的变化,CXCR6^+dMΦ、表面分子CD206的表达明显降低及CD80的表达明显升高,提示CXCR6^+dMΦ可能参与了自然流产的发病机制。Objective:To analyze the proportion and phenotypic differences of decidua CXCR6~+monocytemacrophages in women with normal early pregnancy or unexplained spontaneous abortion.Methods:The decidual tissues were collected from normal early pregnancy women(n=40)and the patients with unexplained spontaneous abortion(n=20).The decidual immune cell populations were separated and the proportion of mononuclear macrophages was analyzed by flow cytometry in normal early pregnancy group and unexplained spontaneous abortion group.The differences in the expression of CXCR6~+,CD80,and CD206 on the surface of decidual mononuclear macrophages(dMΦ)were compared between the two groups.Results:The proportion of CD14+dMΦ in the decidual immune cell population(DIC)in theunexplained spontaneous abortion group was(9.9±1.63)% ,and the proportion of CD14+dMΦin DIC in the normal early pregnancy group was(8.9±0.88)% .There was no significant difference between the two groups(P〈0.05).The expression rate of CXCR6~+in dMΦin normal early pregnancy group[(40.0±1.90)% ]was significantly higher than that in unexplained spontaneous abortion group[(31.22±2.38)% ](P〈0.05).The expression of CD206 and CD80 in CXCR6~+dMΦ was significantly higher than that in CXCR6 "dMΦin normal early pregnancy group(P〈0.05).The expression of CD206 in CXCR6~+dMΦ was significantl" y higher than that in CXCR6 dMΦin unexplained spontaneous abortion group(P〈0.01),but CD80 had no significant change(P〉0.05).Compared with normal early pregnancy group,the expression of CD206 in CXCR6~+/"dMΦ was significantly decreased in the unexplained spontaneous abortion group(P〈0.05).The expression of CD80 in CXCR6" dMΦwas significantly increased(P〈0.05),and there was no significant difference in the expression of CXCR6~+dMΦ(P〉0.05).Conclusions:There was no significant change in the number of decidual mononuclear macrophages among women with unexplained spontaneous
