重症肺炎患者降钙素原、C反应蛋白、外周血白细胞计数及前清蛋白的表达及意义

摘　　要：目的:分析降钙素原(PCT)、C反应蛋白(CRP)、外周血白细胞计数(WBC)及前清蛋白(PA)在重症肺炎中的表达变化及临床意义。方法:161例肺炎分为轻症组(104例)及重症组(57例),并以同期来院进行健康体检的健康者60例作为对照组。对3组的一般资料及PCT、CRP、WBC、PA水平进行比较分析。结果:重症组年龄及男性比例均高于对照组与轻症组(P<0.05);肺炎两组与对照组比较,PCT、CRP、WBC表达显著升高,而PA表达显著降低(P<0.05);而重症组PCT、CRP、WBC表达则显著高于轻症组,而PA的表达则显著低于轻症组(P<0.05)。在四项指标中,PCT拥有较高的灵敏度、特异度和准确度,具备重要的诊断价值。结论:重度肺炎的早期诊断中,PCT拥有较高的特异度、准确度及灵敏度,可作为重症肺炎的早期诊断的重要指标,而PCT、CRP、WBC三项指标联合检测有利于更准确地诊断重症肺炎。Objective：To analyze expression change and clinical significance of procalcitonin（PCT）and c-reactive protein（CRP）,peripheral white blood cell count（WBC）and prealbumin（PA）in severe pneumonia.Methods：A total of 161 cases of pneumonia were divided into mild group（n=104）and severe group（n=57）,and 60 healthy subjects with healthy physical examination in the same period were treated as control group.The general data and PCT,CRP,WBC and PA level of 3 groups were compared and analyzed.Results：The age of the severe group as well as the proportion of males in severe group were higher than those in the control group and the mild group（P〈0.05）.Compared with the control group,the expressions of PCT,CRP and WBC in mild group and severe group were significantly increased,while the expression of PA was significantly decreased（P〈0.05）.The expression of PCT,CRP and WBC in the severe group was significantly higher than that in the mild group,while the expression of PA was significantly lower than that in the mild group（P〈0.05）.Among the four indexes,PCT has high sensitivity,specificity and accuracy,and has important diagnostic value.Conclusion：For early diagnosis of severe pneumonia,PCT has higher specificity,accuracy and sensitivity;hence,it can be used as one of the important indices for the early diagnosis of severe pneumonia.Besides,joint detection of PCT,CRP,and WBC is conducive to a more accurate diagnosis of severe pneumonia.
