摘 要:目的:探讨依达拉奉联合脑苷肌肽辅助治疗对重症颅脑损伤患者神经功能损伤程度的影响。方法:收集在本院接受治疗的重症颅脑损伤患者68例,按照随机数表法分为对照组及观察组各34例,对照组接受常规+脑苷肌肽治疗,观察组接受常规+脑苷肌肽+依达拉奉治疗。对比两组治疗前后血清中神经损伤指标、氧化应激指标、炎症介质含量的差异。结果:治疗前,两组血清中神经损伤指标、氧化应激指标、炎症介质含量的差异无统计学意义(P>0.05)。治疗后,观察组血清中神经损伤指标BNP的含量高于对照组,S-100B、GFAP、NSE的含量低于对照组;血清中氧化应激指标MDA、MPO的含量低于对照组,SOD、GSH-Px的含量高于对照组;血清中炎症介质IL-6、IL-8、TNF-α的含量低于对照组(P<0.05)。结论:依达拉奉联合脑苷肌肽辅助治疗可有效减轻重症颅脑损伤患者的神经功能损害,具体机制与其降低机体氧化应激及炎症反应相关。Objective:To study the effect of edaravone combined with cattle encephalon glycoside and ignotin injection adjuvant therapy on nerve function impairment in patients with severe craniocerebral injury.Methods:A total of 68 patients with severe craniocerebral injury who were treated in our hospital between January 2013 and May 2016 were collected and divided into control group and observation group according to the random number table,34 cases in each group.Control group of patients received conventional+cattle encephalon glycoside and ignotin injection therapy,and observation group of patients received conventional+cattle encephalon glycoside and ignotin injection+ edaravone therapy.The differences in serum levels of nerve injury indexes,oxidative stress indexes and inflammatory mediators were compared between the two groups of patients before and after treatment.Results:Before treatment,serum levels of nerve injury indexes,oxidative stress indexes and inflammatory mediators were not statistically significant between two groups of patients(P 〉0.05).After treatment,serum nerve injury index BNP level in observation group was higher than that in control group while S-100 B,GFAP and NSE levels were lower than those in control group;serum oxidative stress indicators MDA and MPO levels were lower than those in control group while and SOD and GSH-Px levels were higher than those in control group;serum inflammatory mediators IL-6,IL-8 and TNF-αlevels were lower than those in control group(P〈0.05).Conclusion:Edaravone combined with cattle encephalon glycoside and ignotin injection adjuvant therapy can effectively reduce the nerve function impairment in patients with severe craniocerebral injury,and the specific mechanisms are related to its effect on reducing the oxidative stress and inflammation.
