摘 要:目的观察基质金属蛋白酶-9(MNP-9)在急性颅脑患者血清中的表达情况,并探讨其意义。方法收集2015年3月至2017年3月我院收治的62例急性颅脑损伤患者,根据患者入院时的格拉斯哥昏迷评分(GCS),将患者分为轻、中、重、特重型颅脑损伤组。所有患者伤后不同时间点采集外周血,并通过酶联免疫吸附试验(ELISA)法分别检测MMP-9的表达情况。同时收集20名健康志愿者外周血作为对照组。通过统计学分析探讨MMP-9在急性颅脑损伤患者中的表达及意义。结果相比健康对照组,急性颅脑损伤患者血清中MMP-9表达水平升高,且随着损伤程度升高进一步升高(P<0.05);同时动态检测结果显示MMP-9在伤后3~4 d达到峰值,随后逐渐下降;MMP-9脑干损伤及弥漫性轴索损伤中表达较高,而在硬膜外血肿患者表达较低。结论 MMP-9在急性颅脑损伤患者外周血中表达升高,且与颅脑损伤程度呈正相关,可用来作为急性颅脑损伤早期的特异性标志物,同时可以区分不同类型的颅脑损伤。Objective To determine the expression of matrix metalloproteinase-9(MMP-9) in the serum of patients with acute traumatic brain injury, and to investigate its significance. Methods Sixty-two patients with acute traumatic brain injury treated in our hospital between March 2015 and March 2017, were included in the study. Ac-cording to the scores of Glasgow Coma Scale(GCS), all the patients were divided into the mild, moderate and severe and extremely severe traumatic brain injury groups. Peripheral blood was collected from all patients at different time points after injury, and the expression of MMP-9 was determined by ELISA. At the same time, peripheral blood of 20 healthy volunteers was included as the control group. The expression of MMP-9 in patients with acute traumatic brain injury and its significance was statistically analyzed. Results Compared with the healthy control group, the serum MMP-9 level in the patients with acute traumatic brain injury increased, and further increased along with the higher degree of injury(P〈0.05). Meanwhile, the findings of dynamic test showed that the expression level of MMP-9 peaked at 3-4 d after the injury, and then gradually decreased. The expression of MMP-9 in patients with brain stem and diffuse axonal injuries was higher than that in patients with epidural hematoma. Conclusion The expression of MMP-9 in peripheral blood of patients with acute traumatic brain injury increases, and is positively correlated with the injury degree of traumatic brain injury. It can be used as a specific marker of early acute traumatic brain injury, and to dis-tinguish different types of traumatic brain injury.
