摘　　要：目的分析布鲁杆菌病中枢神经系统感染特征,探讨其临床诊断价值。方法选择2014年12月至2016年12月黑龙江省农垦总局总医院住院治疗的急性布鲁杆菌病并发中枢神经损害50例患者作为观察对象。临床诊断并发脑膜炎30例(60.00%)、脑膜脑炎11例(22.00%)、脊髓炎7例(14.00%),脑脓肿2例(4.00%)。调查其住院病例资料,观察临床表现和体格检查及脑脊液(CSF)常规检查各项指标,分析感染临床特点。结果发热50例(100.00%),头痛40例(80.00%),恶心30例(60.00%),意识障碍2例(4.00%),四肢感觉异常7例(14.00%),脑膜刺激征40例(80.00%),癫痫5例(10.00%),脑脊液检查颅内压增高30例(60.00%),白细胞计数增多45例(90.00%)。结论布鲁杆菌病诊断明确者,出现头痛,查体脑膜刺激征阳性,脑脊液白细胞增多,可以初步诊断为布鲁杆菌颅内感染。Objective To analyze the clinical characteristics of patients with Brucellosis concurrent central nervous system infection and explore its contribution to clinical diagnosis. Methods 50 patients with brucellosis central nervous system infection are reviewed. They are hospitalized in General Hospital of Heilongjiang Land Reclamation Bureau from December 2014 to December 2016. Their medical records and the changes of clinical manifestation and physical examination are investigated and analyzed by observing and examination. Clinical diagnosis： thirty cases（60.00%） were Brucellosis meningitis, eleven cases（22.00%） were meningoencephalitis,seven cases（35.00%） were myelitis and two cases（4.00%） were brain abscess. Results 50 cases of fever（100.00%）, forty cases of headache（80.00%）, thirty cases of nausea（60.00%）, two cases of conscious disturbance（4.00%）, five cases of epilepsy（10.00%）, seven cases of abnormal sensation of extremities（14.00%）,forty cases of meningeal irritation sign（80.00%）. Cerebrospinal fluid abnormalities were 48 cases（96.00%） of which 30 cases of increased intracranial pressure（60.00%）, while blood cell count increased were 45 cases（90.00%）. Conclusion Definite diagnosis of Brucellosis concurrent which shows headache, positive meningeal irritation sign and cerebrospinal fluid increases can preliminarily diagnosed with Brucellosis concurrent central nervous system infection.
