摘  要:目的探讨血清25羟基维生素D[25(OH)D]与高龄男性患者下尿路症状(LUTS)的相关性。方法选择在本院住院的年龄≥80岁男性LUTS患者205例,按年龄分为3组,80~85岁组、86~90岁组和>90岁组。测量患者糖化血红蛋白、血脂、肾功、血钙、前列腺特异抗原及25(OH)D等相关指标;超声检查计算前列腺体积(PV),采用国际前列腺症状评分法(IPSS)评估患者下尿路梗阻症状;采用生活质量测定量表(QOL)评估疾病对患者生活质量的影响;尿动力学检查检测排尿期膀胱逼尿肌压力(Pdet)。对3组相关数据进行统计学分析。按照25(OH)D水平正常与否将患者分为正常组与降低组(≥50 nmol/L为正常组,<50 nmol/L为降低组)两个亚组,分别对各年龄组两亚组LUTS相关数据进行对比。结果随着年龄的增长,患者血清25(OH)D水平随之降低(P<0.05),IPSS评分随着增高(P>0.05),Pdet降低(P<0.05)。25(OH)D降低组QOL、IPSS值显著升高,Pdet值降低(P<0.05)。结论 25(OH)D水平与高龄男性患者年龄相关,25(OH)D水平的下降可能参与了前列腺增生及膀胱逼尿肌收缩力下降而导致的LUTS的发生。Objective To explore the correlation of 25(OH)D and lower urinary tract symptoms(LUTS) among the senile men.Methods A total of 205 male patients aged ≥ 80 who had been suffering from of LUTS and had been admitted into the hospital were selected and divided into three groups: the 80-85 years old group, the 86-90 years old group and the 90 years old group. The glycated hemoglobin, blood fat, renal function, blood calcium, PSA, 25(OH)D and other relevant indices were measured. The ultrasonic method was adopted to calculate the prostate volume(PV) and the International Prostate Symptom Score(IPSS) method was adopted to assess the lower urinary tract obstruction(LUTO) symptoms of the patients. The Quality of Life(QOL) scale was used to assess the impact of the disease on quality of life of the patients. The urodynamic study was conducted to detect pressure of detrusor of bladder(Pdet) in urinating phase. Statistical analyses on the data on the patients in the three groups were conducted. The patients were categorized into a normal subgroup(25(OH)D ≥ 50 nmol/L) and a drop subgroup(25(OH)D 50 nmol/L) according to the 25(OH)D levels, and comparisons in LUTS-related indices were made between the two subgroups. Results With the age increasing, the 25(OH)D levels of the patients decreased(P〈0.05), the IPSS scores rose(P〉0.05) and the Pdetvalues dropped(P〈0.05). As for the drop subgroup, the QOL and IPSS values increased significantly while the Pdetvalues decreased greatly(P〈0.05). Conclusion The 25(OH)D level is related to the age of senile men and the drop of which may attributed to LUTS as the result of BPH and reduced contractile force of the detrusor of bladder.
