预变性自体神经移植桥接舌下神经-面神经“侧”-侧吻合术治疗CPA区肿瘤切除术后面瘫患者的临床研究

摘　　要：目的探讨预变性自体神经移植桥接舌下神经-面神经"侧"-侧吻合术在CPA区肿瘤切除术后面瘫中的应用效果。方法选取我院2015年10月—2017年6月CPA区肿瘤切除术后面瘫患者62例,随机数字表法分为研究组(n=31)与对照组(n=31)。对照组采取面部功能训练,研究组采取预变性自体神经移植桥接舌下神经-面神经"侧"-侧吻合术+面部功能训练。统计两组治疗效果、治疗前后鼻尖偏离度。结果研究组总有效率93.55%(29/31)高于对照组41.94%(13/31)(P<0.05);治疗前两组鼻尖偏离度间无显著差异(P>0.05),治疗后两组鼻尖偏离度较治疗前增大,且研究组大于对照组(P<0.05)。结论采取预变性自体神经移植桥接舌下神经-面神经"侧"-侧吻合术治疗CPA区肿瘤切除术后面瘫,可提高疾病治疗效果,改善鼻尖偏离度。Objective To investigate the application effect of hypoglossal-facial ‘side＇-to-side neurorrhaphy using a predegenerated sural nerve autograft in the treatment of facial paralysis after tumor resection in CPA region. Methods A total of 62 cases of facial paralysis after tumor resection in CPA region in our hospital from October 2015 to June 2017 were randomly divided into study group（n=31） and control group（n=31）. The control group received facial function training, while the study group adopted hypoglossal-facial ‘side＇-to-side neurorrhaphy using a predegenerated sural nerve autograft ＋ facial function training. The treatment effect and the deviation degree of the nasal tip before and after treatment were statistically analyzed between the two groups. Results The total effective rate in the study group was 93.55%（29/31）, which was higher than that in the control group 41.94%（13/31）（P〈0.05）. There was insignificant difference between the two groups in the deviation degree of the nasal tip before treatment（P〈0.05）. After treatment, the deviations of the nasal tip in the two groups increased compared with those before treatment, and the study group was larger than the control group（P〈0.05）. Conclusion The hypoglossal-facial ‘side＇-to-side neurorrhaphy using a predegenerated sural nerve autograft in the treatment of facial paralysis after tumor resection in CPA region can improve the effect of disease treatment and improve the deviation of nasal tip.
