富血小板血浆治疗老年糖尿病患者褥疮的疗效观察

摘　　要：目的:探讨富血小板血浆(PRP)治疗老年糖尿病患者褥疮的临床疗效。方法:选取该院2013年6月—2015年7月收治的糖尿病并发褥疮患者69例,将其随机分为PRP治疗组(34例)和对照组(35例),PRP治疗组采用PRP进行治疗,对照组采用传统方法治疗。2组患者均使用胰岛素控制血糖,并严格控制血压及血脂,同时予抗血小板聚集、改善微循环及抗感染等一般治疗。观察2组患者创面愈合情况,比较愈合时间及有效率。结果:所有患者均完成12周随访。两组患者年龄、性别、糖尿病史、住院时间及褥疮等基本情况相比差异均无统计学意义(P均>0.05)。PRP治疗组患者的治疗时间和褥疮创面愈合时间均显著短于对照组(P均<0.05)。PRP治疗组有效率为82.4%,显著高于对照组(P<0.05)。结论:PRP对治疗老年糖尿病并发褥疮患者有较好的临床疗效,且治疗周期短及治愈快,值得临床进一步推广应用。Objective： To evaluate the clinical effects of platelet-rich plasma （PRP） in treating diabetic pressure ulcer. Methods： From June 2013 to July 2015, 69 inpatients with diabetic pressure ulcer were randomly divided into two groups. PRP group （34 cases） was treated with PRP, while the control group（35 eases） was given conventional treatments. The blood sugar levels were controlled by insulin. Blood pressure and circulating levels of lipids were well-controlled in all patients. Both groups were also given anti-platelet, improvements of microeirculation and anti-infection treatments. Wound healing time and effective rates were compared between the two groups. All patients were followed up for 12 weeks. Results： Prior to treatment, age, gender, diabetic history, hospitalization time and pressure ulcer were no significant differences between the two groups （P all 〉0.05）. Both treatment and healing times were significant shorter in PRP than in control group （P all〈0.05）. The effective rates of PRP group（82.4%） were significant higher than that of control group（P〈0.05）. Conclusions： PRP is su- perior to conventional methods for treating diabetic pressure ulcer. It is worthy to try.
