摘  要:目的比较低温等离子刀与高频电刀在混合痔外剥内扎术中的疗效,分析二者的特点和应用价值。方法将160例混合痔Ⅲ期和Ⅳ期患者随机分为低温等离子刀组及高频电刀组,每组各80例。低温等离子刀组采用传统外剥内扎联合低温等离子刀术,高频电刀组采用传统外剥内扎联合高频电刀术。观察两组患者的手术时间、术中出血量、切口愈合时间,以及肛门疼痛、肛门水肿和瘢痕评分。结果低温等离子刀组的手术时间和术中出血量分别为(24.50±6.03)min、(12.46±5.73)m L,均显著多于高频电刀组的(19.10±5.75)min、(9.41±3.47)m L,差异有统计学意义(均P<0.05)。低温等离子刀组在术后第1、3、7天的肛门疼痛评分,以及肛门水肿评分、瘢痕评分分别为(3.54±1.93)分、(3.08±1.57)分、(2.51±1.07)分、(0.61±0.72)分、(6.64±1.88)分,均显著低于高频电刀组的(4.35±1.91)分、(3.68±1.64)分、(3.19±1.59)分、(1.26±0.88)分、(7.50±2.05)分,差异有统计学意义(均P<0.05)。低温等离子刀组和高频电刀组的切口愈合时间比较,差异无统计学意义(P>0.05)。结论在进行混合痔外剥内扎手术时,低温等离子刀的应用可以减少术后并发症,有利于患者术后恢复。Objective To compare the curative effects of low-temperature plasma cutter and high frequency electric knife in Milligan-Morgan hemorrhoidectomy for mixed hemorrhoids, and to analyze the characteristics and application value of both methods. Methods 160 eases of patients with HI and IV stage mixed hemorrhoids were randomly divided into two groups, with 80 cases in each group. The low-temperature plasma cutter group was treated with traditional Milligan-Morgan hemorrhoidectomy combined with low-temperature plasma cutter surgery, while the high frequency electric knife group was treated with Milligan-Morgan hemorrhoidectomy combined with high frequency electric knife surgery. The operation time, amount of intraoperative blood loss, incision healing time, and anal pain, anal edema and scar scores of two groups were observed. Results The operation time and amount ofintraoperative blood loss of low-temperature plasma cutter group were (24.50 ± 6.03) min and (12.46 ± 5.73) mL respectively, significantly higher than (19.10 ± 5.75) min and (9.41 ± 3.47) mL of high frequency electric knife group (P〈0.05). The anal pain scores at 1st, 3td and 7th day after surgery, anal edema score and scar score of the low-temperature plasma cutter group were (3.54 ± 1.93) points, (3.08 ± 1.57) points, (2.51 ± 1.07) points, (0.61 ± 0.72) points and (6.64 ± 1.88) points respectively, significantly lower than (4.35 ± 1.91) points, (3.68 ± 1.64) points, (3.19 ± 1.59) points, (1.26 ± 0.88) points and (7.50 ± 2.05) points of the high frequency electric knife group (P〈0.05). No statistical difference was found between the two groups in the incision healing time (P〉0.05). Conclusions The applicationoflow-temperatureplasmaeutterinMiUigan-Morganhemorrhoidectomy for mixed hemorrhoids can reduce the postoperative complications, which is conducive to postoperative recovery of patients.
